— — PAGE 1— 摘 要 超声对腮腺常见良性肿瘤的诊断价值 研究生：陈凤志 导师：娜迪热·铁列吾汗 副教授 摘 要 目的：探讨超声对腮腺多形性腺瘤与Warthin瘤的诊断依据，旨在不断提高其诊 断符合率。方法：回顾性分析经手术及病理证实的12例腮腺多形性腺瘤病例、10例 Warthin瘤病例的二维声像图及彩色多普勒血流显像特点。分析其超声成像中各种特 异性表现的诊断敏感性，从肿瘤的大小、个数、边界、包膜、内部回声及后壁回声 等指标做综合分析。结果：多形性腺瘤病例组，超声诊断为多形性腺瘤6例(50%)， 淋巴结炎2例(16.7%)，血肿1例（8.3%），混合性占位、性质待定3例(25%)。腮腺Warthin 瘤病例组，超声诊断为Warthin瘤4例(40%)，多形性腺瘤3例(30%)，淋巴结炎1例(10%)， 混合性占位、性质待定2例(20%)。多形性腺瘤的声像图特征与Warthin瘤有一定的相 似性。结论：典型的多形性腺瘤、Warthin瘤有各自的声像图特点，非典型的多形性 腺瘤类似于Warthin瘤。认为超声检测多形性腺瘤、Warthin瘤有较高的临床价值，要 做到病理学诊断有一定难度，但它对临床医师的诊断和治疗有直接指导。 关键词：多形性腺瘤；超声诊断；Warthin 瘤；彩色多普勒血流显像 新疆医科大学硕士学位论文 Ultrasonic Diagnosis and Analysis of Parotid Gland Benign Tumor Postgraduate:Chen fengzhi Tutor:Na Di-Re vice profseeor (The First Affiliated Hospital of Xinjiang Medical University,Urumqi 830054,China) Abstract Objective: To promote the diagnostic comforming rate of pleomorphic adenoma and warthin tumor through the research on the evidence for ultrasonic diagnosis of the disease Methods: Through retrospective analysis of ultrasound imaging characteristics under 2D and CDFI, 22 patients were proved by operation and pathological method. They have been synthetically analyzed through volume, number, hardness and internal echoes of the tumor. Result: 12 cases pleomorpic adenoma,There were 6 cases with pleomorpic adenoma, 2 cases with lymph adenitis and 3 cases with pleomorphic mass of unkown origin. Fifty percent of the cases under ultrasonic diagnosis were comfirmed by pathologic findings.10 cases warthin tumors, There were 4 cases with warthin tumors, 3 cases w ith pleomorphic adenoma, 1 cases with lymph adenitis and 2 cases with pleomorphic mass of unkown origin.
