关节镜治疗交叉韧带断裂16例的的疗效观察 　　摘 要 目的：探讨关节镜下治疗膝关节前/后交叉韧带断裂的临床治疗效果。方法：2008年2月～2010年12月收治交叉韧带断裂患者16例，其中男12例，女4例，前交叉韧带断裂14例，后交叉韧带断裂1例，前后交叉韧带同时断裂1例，5例伴外侧半月板损伤，2例伴内侧半月板损伤，2例内外侧半月板同时损伤。本组患者均有明显外伤史。关节疼痛12例，行走不稳13例，关节交锁3例，不能慢跑14例，术前MRI检查均提示ACL/PCL断裂。治疗时间在伤后2～20周，采用关节镜下自体?绳肌肌腱单束四股移植重建前后交叉韧带。术前Lysholm评分为46～62分，平均50分，详细记录术后1个月、6个月、1年、2年的患者状况。结果：术后均随访2～24个月，平均16个月，术前Lysholm评分平均50分，术后提高到92分，优良率达87.5%。结论：关节镜下治疗交叉韧带断裂，创伤小，疗效理想，稳定好，并发症少，早期功能训练预后效果理想。 　　关键词 关节镜 交叉韧带断裂 ?绳肌肌腱 重建 疗效 　　 　　AbstractObjective:To study the treatment arthroscopic knee after anterior/posterior cruciate ligament rupture of clinical treatment effect.Methods:To analyse from 2008 to 2010 December follow-up data whole 16 cases of patients,including cruciate ligament rupture male 12 cases,female 4 cases,anterior cruciate ligament rupture 14 cases,after cruciate ligament rupture,1 case before and after the cruciate ligament while fracture,5 patients with the lateral meniscus damage,2 cases with the medial meniscus damage,2 cases within the lateral meniscus also damage.All of the patients are obviously injury history,clinical:joint pain in 12 cases,walking instability 13 cases,joint hand in 3 cases,cant lock jogging 14 cases,preoperative MRI strongly suggest that ACL/PCL fracture.Time to treatment in injury after 2～20 weeks,using arthroscopic popliteal rope muscle tendon autograft single beam four shares graft reconstruction around cruciate ligament.Preoperative Lysholm score 46～62 points,average 50,detailed records postoperative afte month,6 months,1 year,2 years of patient condition.Results:After follow-up 2～24 months,averaging 16 months, preoperative Lysholm score averaged 50,and postoperative increased to 92,the better rate is87.5%.Conclusion:Arthroscopic treatment cruciate ligament rupture,small trauma,curative effect is ideal,stable,fewer complications,early functional training prognosis results are ideal. 　　Key WordsArthroscopy;Cruciate ligament rupture popliteal;Rope muscle tendon;Reconstruction;Curative effect
