失效模式和效应分析在」CI认证医院手术室工勤人员洗手依从性中的应用 蒋旭温丽婷通信 南京医科大学附属无锡市第二人民医院江苏无锡214002 】目的：通过失效模式与效应分析(FMEA)探讨手术室工勤人员洗手 依从性及认知情况，并分析存在问题。方法：采用FMEA法针对手术室工勤人员 洗手依从性低的问题组成团队，进行失效模式与潜在风险原因分析，计算事先的 风险指数(RPNK#,针对高风险指数进行优先处理，制定改善措施及实施，比较 活动前后RPN值，手术室工勤人员对手卫生知识知晓率、洗手执行率和准确率， 从而确认活动效果。结果：实施FMEA后事先风险指数有明显的卜降，原RPN 值2092分，后RPN值309分，不降了 85.2% ,手卫生知识知晓率、洗手执行 率和准确率均有提高，差异有统计学意义(Plt;0. 05)。结论：FMEA方法的应用 可有效提高手术室工勤人员手卫生知识，提高洗手依从性，杜绝引起院感的隐患, 并且培养了科室的凝聚力，提高了护士解决问题的能力。 关键词】失效模式与效应分析；KI认证医院；手术室工勤人员；洗手的 依从性 】R192 】B 】1674-8999 ( 2015) 8-0132-02 Abstract】 Objective Through the failure mode and effect analysis (FMEA) to discuss the operating room those logistics staff hand washing compliance and cognitive situation, and analyze the existing problems. Methods The FMEA method to counter the problem of lower operating room those logistics staff hand washing compliance team, analyzing the causes of failure modes and the potential risk, risk index (RPN) value of the calculation in advance, high risk index for priority, make improvement measures and implementation, compared before and after RPN value activities, operating room those logistics personnel aware of knowledge about health and hands are enforced and accuracy, thus confirming the effect. Results After the implementation of FMEA index has obvious reduction in the risk in advance, the original RPN value of 2092 points, after the RPN value of 309 points, down 85.2%, hand hygiene knowledge awareness and hands are enforced and accuracy were increased, the difference was statistically significant (P lt; 0.05). Conclusion The application of FMEA method can effectively improve the operating room those logistics personnel hand hygiene knowledge, enhance adherence to wash your hands, put an end to cause school feeling hidden trouble, and cultivate the department cohesion, improve the nurses ability to solve problems.
