门诊1645例不合理中成药处方干预分析及的探讨 　　[摘要] 目的 通过对门诊不合理中成药处方实施事前干预措施，促进临床合理使用中成药，规范处方书写，保障患者用药安全有效。 方法 对2012年10月～2013年10月北京积水潭医院（以下简称“我院”）门诊中药房干预的1645张不合理处方进行分析。 结果 2012年10月～2013年10月我院门诊不合理处方干预质量和数量逐步提高，不合理用药情况复杂，其中以用法与用量不规范、联合用药不合理及药物规格和数量录入错误较常见，主要为医师未签字或与签字卡不符、处方颜色用错、跨科开药、超说明书最大用量未签字、超限量开药、疑似重复用药、联用不合理、超适应证或禁忌证用药、诊断与用药不符或缺少诊断、用法错误。 结论 不合理处方审方干预能及时而有效地保障患者的用药安全，应不断改进和完善不合理处方审方干预的各项措施，促使医师和药师提升中药辨证论治能力，保障处方质量和规范合理使用中成药。 　　[关键词] 不合理处方；处方干预；事前干预；处方审核；用药分析 　　[中图分类号] R969.3 [文献标识码] C [文章编号] 1673-7210（2014）10（b）-0154-05 　　Analysis of 1645 cases irrational outpatient Chinese medicine prescriptions intervention 　　LIU Ran XING Shuang WANG Lu 　　Department of Pharmacy， Beijing Jishuitan Hospital， Beijing 100035， China 　　[Abstract] Objective Through the implementation of a hospital irrational outpatient Chinese medicine prescriptions pre-intervention measures， to regulate prescription writing， promote and guide the rational use of medicine， protect the safety of patients， and effective. Methods From October 2012 to October 2013， in Beijing Jishuitan Hospital （our hospital for short）， 1645 cases of pharmacist interven irrational prescription were analyzed retrospectively. Results From October 2012 to October 2013 in our hospital outpatient prescriptions， the quality and quantity of unreasonable prescription intervention showed increased gradually， unreasonable prescription were complex and diverse， inappropriate usage and dosage， inappropriate combination therapy， drugs specification， entry errors were common ， mainly in the physician without signature or consistent cards， prescription with the wrong color， interdisciplinary prescription， over-the maximum dosage without signature， beyond limited dosage， suspected of repeated drug prescription， unreasonable combinatoin， beyond indications or contraindications， diagnosis and medication discrepancies or missing aspects of diagnosis， usage errors. Conclusion The timely and effective intervent irrational prescription manner can protect
