原发结外非霍奇金淋巴瘤110例临床剖析 　　【摘 要】目的：探究原?l结外非霍奇金淋巴瘤的临床特点、诊断、治疗。方法：选取2012年4月-2015年4月我院收治的110例原发结外非霍奇金淋巴瘤患者作为研究对象，进行回顾性分析。结果：110例非霍奇金淋巴瘤，原发于胃肠道26例，占23.6%；Waldeyer环14例，占12.7%；鼻腔和软组织各10例，各占9.10%；纵膈10例，占9.10%；骨和颅内各8例，占7.27%。其余腮腺6例，皮肤5例，脾脏5例，甲状腺4例，眼部4例。原发器官肿大或有肿块88例，占80%；腹痛12例，腹部包块10例，食欲减退7例，腹部不适4例，肠梗阻3例，血便2例；干咳10例，胸闷9例；咽痛有异物感9例；头痛、头晕4例，鼻塞7例。110例患者中，B症状35例：消瘦13例，发热10例，盗汗12例；血清LDH升高41例，占37.2%。91例患者进行骨髓检查，骨髓浸润26例。占28.2%；对91例患者进行临床分期及国际预后指数评分：Ⅲ/Ⅳ期33例36.2%、低、中、危组分别占38例、42例、11例，分别占41.7%、46.1%、12.1%。结论：原发结外非霍奇金淋巴瘤首发部位广泛，临床表现没有特异性，造成诊断困难，易出现误诊、漏诊情况。对原发结外非霍奇金淋巴瘤具备全面的认识，并结合放化疗结合的治疗方法，是提高生存率，延缓病情发展的关键。 　　【关键词】原发结外非霍奇金淋巴瘤；恶性淋巴瘤；诊断；治疗 　　Abstract Objective： To explore the clinical features， diagnosis and treatment of primary non-hodgkins lymphoma. Methods： a retrospective analysis was made of 110 patients with non-hodgkins lymphoma who were admitted to our hospital from April 2012 to April 2015. Results： 110 cases of non-hodgkins lymphoma were found in 26 cases of gastrointestinal tract， accounting for 23.6%. Waldeyer ring 14 cases， accounting for 12.7%； The nasal cavity and soft tissue were 10 cases， each accounting for 9.10%. Mediastinal 10 cases， accounting for 9.10%； There were 8 cases of bone and skull， accounting for 7.27%. There were 6 cases of parotid gland， 5 cases of skin， 5 cases of spleen， 4 cases of thyroid and 4 eyes. There were 88 cases of swelling or swelling of primary organs， accounting for 80%. There were 12 cases of abdominal pain， 10 cases of abdominal pain， 7 cases of anorexia， 4 cases of abdominal discomfort， 3 cases of intestinal obstruction， and 2 cases of blood. 10 cases of dry cough and 9 cases of chest tightness； There were 9 cases of foreign body sensation in pharynx. Headache， dizziness 4 cases， nasal obstruction 7 cases. Among 110 patients， 35 cases of B were： 13 cases of wasting， 10 cases of fever and 12 cases of night sweats.
