区镇一体化紧密型医联体运行效果剖析 　　[摘要] 目的 评估区镇一体化紧密型医联体的运行效果，为医联体的推广提供参考。 方法 2013年9月起，以绍兴第二医院为核心构筑医联体。随后对比评价医联体前、医联体后1年、医联体后2年的双向转诊情况、业务收入、业务结构变化及患者和医务人员对诊疗体系运营的满意度。 结果 医联体后1年、2年的手术人次、业务收入、门诊量等指标较医联体前均有明显升高，差异具有统计学意义（P0.05）。医联体后1年、医联体后2年的转诊患者数量、门诊预约患者数量、下转患者数量等较医联体前均明显增多，差异具有统计学意义（P0.05）。医联体后1年、医联体后2年，职工和患者对医疗服务体系的满意度均明显升高，与医联体前比较差异具有统计学意义（P0.05）。 结论 区镇一体化紧密型医联体的实施能有效优化医疗资源的使用，提高医疗服务的质量，值得推广。 　　[关键词] 区镇紧密型一体化；医联体；转诊率；运营情况 　　[中图分类号] R197.3 [文献标识码] C [文章编号] 1673-9701（2016）23-0125-04 　　[Abstract] Objective To evaluate the operation effect of district-town integrated close medical consortium and provide reference for its promotion. Methods From September 2013， the medical consortium with Shaoxing Second Hospital as the core was established. Then the two-way referral situation， business income， business structure changes and satisfaction of patients and medical staffs to the operation of diagnosis and treatment system before the establishment of medical consortium and 1 year and 2 years after the establishment of medical consortium were compared. Results One year and two years after the establishment of medical consortium， indicators such as surgical person-time， business income and outpatient quantity significantly increased compared to those before the establishment， with statistically significant differences（P0.05）. One year and two years after the establishment of medical consortium， the number of referral patients， number of outpatient appointment patients and number of down-referral patients significantly elevated compared to those before the establishment， with statistically significant differences （P0.05）. One year and two years after the establishment of medical consortium， satisfaction of both medical staffs and patients to the medical service system significantly improved compared to that before the establishment， with statistically significant difference （P0.05）.
