MRI和CT诊断嗅神经母细胞瘤的影像表现探讨 陈志刚雷明军丁梓华阳煜刘小明 湖南省株洲市攸县人民医院放射科湖南攸县412300 】目的：分析MRI和CT诊断嗅祌经母细胞瘤的影像表现。方法: 选取我院收治的11例嗅神经母细胞瘤患者为木次研究对象，所有患者均经过病 理活检组织确诊。对检查影像的病变累及部位、大小，侵入范围，病变密度、信 号强化程度等进行分析。结果：11例患者均有不同程度的骨质被破坏影像，肿 瘤密度不均匀，可以看见斑片状低密度形态，肿瘤形态均不规则。TIWI表现低 于脑灰质的低信号病灶,T2WI信号高于脑组织高信号。8例患者伴有囊变坏死， TIWI低信号不均匀，T2WI高信号不均匀。7例进行增强检查的患者中1例呈现 强化均匀，其余强化不均匀较明显。3例左侧额叶受累，5例右侧额叶受累。结 论：临床患者检查出现鼻腔筛窦、顶部有肿块出现，并且有沿着嗅神经呈现蔓延 趋势，强化显示较为明显等情况时，应重点对患者检查是否为嗅祌经母细胞瘤。 关键词】MRI检查；CT检查；嗅神经母细胞瘤 Study on the MRI and CT images in the diagnosis of olfactory nerve tumor CHEN Zhigang, LEI Mingjun, DINGZihua，YANG Yu, LIU Xiaoming (The radiology department，Youxian Peoplersquo;s Hospital of Zhuzhou City, Hunan, 412300，China) Abstract： Objective： To study the MRI and CT in the diagnosis of olfactory nerve tumor. Methods： We selected 11 cases of olfactory nerve tumor patients treated in our hospitalas the study objects. All patients after pathological biopsy were diagnosed. The image of the lesion involving the location，size，scope of intrusion， the lesion degree of density, signal enhancement is analyzed. Results： 11 cases of patients had different degrees of bone destruction of images, tumor uneven density, patchy low density，irregular tumor shape. TIWI performance was lower than the gray matter of low signal intensity，high T2WI signal was higher than the brain signals. Eight patients had cystic necrosis，uneven TIWI low signal，uneven high T2WI signal. 1 case in 7 patients with enhanced inspection appeared strengthen uniform, and the rest of the reinforcement was apparently uneven. 3 cases of the left frontal lobe had dysfunction, 5 cases of right frontal lobe had dysfunction. Conclusion： patients with clinical examination of ethmoid sinus and nasal cavity with lump appears at the top and along the olfactory nerve showed a trend of spreading，at the situation such as evident strengthening，we should focus on checking for olfactory nerve in patients with tumor.
