腹腔镜广泛子宫切除及盆腔淋巴结清扫术治疗早期子宫颈癌及开腹手术对比研究 】目的比较腹腔镜广泛子宫切除和盆腔淋巴结清 扫术治疗早期宫颈癌与开腹手术的临床疗效及安全性。方法 回顾性分析2008年11月至2010年7月于我院接受腹腔镜 广泛子宫切除和盆腔淋巴结清扫术的40例早期宫颈癌患者 （腹腔镜组）的临床资料，并随机抽取同期行同类开腹手术 的40例患者（开腹组）作为对照组，将两组资料进行对比分 析。结果腹腔镜组的手术时间明显长于开腹组，术中出血 量、术后排气时间均明显少于开腹组，该两组的淋巴结切除 数目、保留尿管时间及术后住院时间等指标均无显著性差 异。腹腔镜组术中输尿管损伤1例，开腹组术中膀胱损伤1 例，腹腔镜组和开腹组的术后并发症率分别为32. 5%、37. 5%， 无显著性差异。80例患者的随访时间为11-32个月，至今尚 无复发病例。结论腹腔镜广泛性子宫切除和盆腔淋巴结清 扫术可作为早期子宫颈癌手术治疗的选择方法之一。 关键词】子宫颈癌腹腔镜广泛子宫切除 分类号:R737. 3 :A 1005-0515 （2011） 12-025-03 A comparison of laparoscopic and abdominal radical hysterectomy JIAO Lanzhou JIANG Jiyong , CAO Rui (and pelvic lymphadenectomy in early stage cervical cancer) [Abstract ] Objective To compare the clinical effectiveness and safety of laparoscopic and abdominal radical hysterectomy and pelvic lymphadenectomy in early stage cervical cancer. Methods A retrospective analysis was carried out with a total of 80 patients with early stage cervical cancer treated in our hospital from November 2008 to July 2010. 40 patients underwent laparoscopic radical hysterectomy and pelvic lymphadenectomy (laparoscopy group), and another 40 patients were treated with abdominal radical hysterectomy and pelvic lymphadenectomy (laparotomy group). Multiple clinical parameters were observed and analyzed statistically. Results Comparing with the patients in laparotomy group, the patients in laparoscopy group had a longer operating time, lower blood loss, and quicker return of bowel function. There were no significant differences in lymph nodes excised, catheter removal time, and hospital stays between the two groups. There was one case of ureteral injury in laparoscopy group and one case of bladder injury in laparotomy group. The rate of postoperative complication was 32. 7% in laparoscopy group and 37. 5% in laparotomy group with no significant differences. There was no case of recurrence during the followup time (11-32 months). Conclusions Laparoscopic radical hysterectomy and pelvic lymphadenectomy is an effective and
