PAGE PAGE VI 改良阔筋膜加强修复与张力带钢丝 减张缝合治疗急性髌韧带损伤对比 中 文 摘 要 目的 ：比较改良设计阔筋膜加强修复与张力带钢丝减张缝合治疗急 性髌韧带实质部“马尾状”损伤的临床治疗效果。 方法 ： 本研究收集 2008 年 6 月至 2014 年 3 月间共 24 例行髌韧带 断裂修复术病例，按改良设计阔筋膜加强修复和张力带钢丝减张缝合分 为治疗组和对照组，其中治疗组 12 例，对照组 12 例。术后对其进行临 床评估(Lysholm 评分、膝关节活动范围、大腿萎缩、X 线片检查评估 Insall-Salvati 髌骨高度)，比较两者临床治疗效果。 结果：治疗组临床效果评估显示，术后 12 个月 Lysholm 评分为 87.13±6.18，对照组为 85.27±6.23，大腿萎缩及膝关节活动范围两组 无明显差异（P﹥0.05）；但 Insall-Salvati 髌骨高度对照组显著小于治疗组 (P0.05)。 结论 ： 相对于张力带钢丝减张缝合技术，改良设计阔筋膜加强修 复技术可以更好维持髌骨正常高度。 关键词： 髌韧带，髌腱，阔筋膜，膝损伤，关节镜 The Comparision Between the Broad Fascia Strengthen Repair that Improved and the Tension Band Steel Wire Cut Suture to Treat Acute Patellar Tendon Injury ABSTRACT Objective: To compare with clinical curative effects of the broad fascia strengthen repair that improved design and the tension band steel wire cut suture to treat acute hail-like essence of patellar tendon injury. Methods: I collected 24 cases that the patients had surgeries of patellar ligament rupture revascularization from June 2008 to March 2014 in this study,according to the broad fascia strengthen repair that improved design and the tension band steel wire cut suture,dividing them into the treatment group and control group,and there are 12 cases in the treatment group,12 cases in the control group.To have clinical evaluation after treatment (Lysholm rating, knee-joint’s the rage of activity,thigh atrophy,X-ray examination evaluates Insall-Salvati patellar height) and compare with clinical curative effects in the two groups. Results: The assessment of clinical curative effect shows that Lysholm rating is 87.13±6.18,85.27±6.23 in the control group,thigh atrophy,no obvious differences and knee-joint’s the rage of activity in the two group (P0.05);but Lachman Insall-Salvati patellar height in the control group are obviously smaller than the treatment group’s (P0.05). Conclusion(s): The broad fascia strengthen repair that improved design can protect maintain patellar normal height better,relating to the tension ban
