2007 6 20 6 Ch ina J Orth op T raum a, Jun. 2007, V o.l 20, No. 6 367 禹宝庆, 张春才, 苏佳灿, 胡海波, 刘辉 ( , 200433) ! : 寻找 一种新的治疗桡骨远端骨折与骨不连的内固定方法: 根据桡骨远端的解剖学特点骨折 发生机 及骨折端的应力情况, 利用镍钛生物记忆材料的特殊力学行为, 设计 一种专门处理此类骨折骨不连的形状记 忆合金接骨器临床上采用该接骨器治疗桡骨远端粉碎性骨折 5例, 桡骨远端骨折骨不连 20例桡骨远端粉碎骨折组 中, 男3例, 女2例; 年龄 44~ 65岁, 平均 513岁骨不连组中, 男 14例, 女6例; 年龄 34~ 70岁, 平均 557岁; 其中采用 普通钢板固定7例, 三棱针固定8例, 克氏针加钢丝固定2例, 外固定架固定 1例, 石膏固定2例: 生物力学测试表 明本接骨器具有良好的耐疲劳与重复使用性, 初期产生的最大环抱持骨力 1282 MPa, 纵向压力 136MPa本组所有患 者均得到随访, 时间6 个月~ 2年, 平均 13年1例出现切口软组织的浅表感染, 经局部换药2周后, 切口愈合其余患 者切口均∀ 期愈合腕关节伸屈活动范围 120#~ 140#, 术后 6~ 10周骨折线消失所有患者术后骨折无成角畸形骨折 再移位出现, 无骨不连发生, 取出聚合器后无再骨折: 桡骨远端解剖型记忆聚合器符合桡骨远端的解剖特点, 可以 在骨折端产生持续纵向加压的作用, 具有抗剪力抗折弯力抗分离抗旋转作用, 可以促进骨折愈合 ! 桡骨骨折; 外固定器; 骨折愈合; 生物力学 Design and clinical applications of Anatom ical N itinolM em ory Connector for distal radius YU B aoqing, ZH ANG Chunca,i SU J iacan, H UH aibo, LIU H u .i D ep artm en t o O rthop aedics, the A iliated Chang haiH o sp ital o the Second M ilitary M ed ical University, Shanghai 200433, Ch ina ABSTRACT Objective: T o find a new method of internal fi ation for them anagem ent of fractures and nonun ion of distal rad i us. M et ods: Accord ing to the anatom ical feature, m echanism of fracture and stress sustained on the fracture s ite of distal rad i us, a new type ofN itinol shapem emory alloy connectorw as designed by tak ing the advantage of its specialm echan ics action. In clin ical applications, 25 patients w ere treated w ith the procedure, amongwh ich 5 patients w ere fresh fracture of distal rad ius and 20 patients w ere nonun ion.
