提 提 要 本研究运用祖国医学理论和中西医结合的方法，从理论、I临床研究两方面对活动 期强直性脊柱炎的病因病机、治法方药进行了较为系统的深入探讨。认为湿热瘀毒、 阻滞肾督是活动期强直性脊柱炎的主要病机，临床以清热解毒利湿活血、补肾壮督为 主要治法，拟定脊痹汤方剂为治疗组，以火把花根片为对照组，共观察60例患者(两 组均为30例)，结果显示：临床控显率(近期控制和显效)治疗组为63．33％，对照 组为26．67％，治疗组明显优于对照组(尸0．01)；总有效率两组无显著性差异(P 0．05)：在改善临床症状、体征、实验室指标、防止骨质破坏等方面治疗组均优于对 照组(尸0．01或P0．05)。结合现代医学及中药药理学分析，其作用机制可能是通 过抗感染、抗炎镇痛、调节免疫功能、改善血液流变性等方面，从多环节对活动期强 直性脊柱炎发挥有效的治疗作用。 关键词强直性脊柱炎：活动性；脊痹汤；临床研究 壹曾箩、导帅蛐麓 ■全文公柑 一 The The clinical study of Jibi Decoction On Ankylosing Spondylitis in active phase Speciality：Internal Medicine ofT．C，M Rheumatic Disease Author：Ju Zhongbin Tutor：Professor Zhang Liting Abstract This stud y’baseing on the theory of TCM and combination of Traditional Chinese with western medicine，Systematically and intensively explores the etiological factors and therapeutic methods for ankylosing spondylitis(AS)in active phase from both theoretical and clinical aspects．The research considers the Wet heat and stasis toxin and the blocking of kidney and Du Mai as the main causes of AS in active Base，and accordingly adopts eliminating Wet heat toxin，activating blood and strengthening kidney and Du Mai as the main therapeutic method．In the study JI BI decoction is taken as the test group with Huo Ba Hua Gen Tableas as control one and with 30 subjects in each group．The results show that the controlled and effective rate in test group is 63．33％with 26．67％in control one(P 0．01)．But the total effective rate shows no difference in two groups．In the aspect of of eliminating symptoms，improving laboratory index and antidestruction of bone，the test group is superior to the control one(P0．0l or尸0．05)．The mechanism might be relate to anti—inflammation，relieving pain，regulating immunological function，improving hemorheological index and bettering microeirculation，etc．In such view，it treats AS effectively in poly-side． Key words ankylosing spondylRis(AS)；active phase；Jibi Decoction；clinical study Y Y 622530 原创性声明 本人郑重声明：所呈交的学位论文，是在导师的指导下独立完成 的，文中除注明引用的内容外，不包含任何其他已经发表的科研成果。
