3oo 中华内分泌代谢杂志2002年8月第18卷第4期 Chin J Endocrinol Metab，August 2002，Vol 18，N[)l 4 ● 临床 研 究 ● 促性腺激素释放激素类似物简易激发试验 对性早熟诊断的评价 马华梅 杜敏联 陈红珊 李燕虹 苏拮 【摘要】 目的 评价促性腺激素释放激素类似物(GnRHa)简易激发试验对性早熟的诊断价 值。方法 乳房发育提前的74例女孩(就诊时行0、30和60 min简易GnRHa激发试验)被分为4 组：真性性早熟(CPP)B：组 l9例；CPP B3组23例；单纯性乳房早发育(PT)组22例；外周性性早熟 (PPP)组l0例。对各组激发试验进行比较和分析。结果 PPP组3个时相血清FSH及LH浓度差异 无显著性。PT、CPP B2和CPP B3组3个时相的血清FSH及LH浓度差异均有显著性(P0．01)。PT 组AFSH(8．0±3．2)IU／L显著高于ALH[(1．9±1．4)IU／L，P0．01]。CPP B2组AFSH(7．0±2．9) IU／L与ALH(8．2±6．7)IU／L差异无显著性。CPP B 组 △LH(23．0±22．0)IU／L显著高于 AFSH [(9．0±4．3)IU／L，P0．05]。PT、CPP B2和CPP B3组的ALH及峰LH／FSH[(0．4±0．2)、(1．1 ±0．8)和(1．9±1．7)]差异有显著性(P0．01)。以LH峰值I4．4 IU／L和峰LH／FSH≥0．46为前 提，诊断CPP的灵敏度和特异度分别为90．5％和90．9％。结论 PPP组垂体对 GnRHa无应答。PT 组反应以FSH升高为主，CPP B 和CPP B 组以LH升高为主。此激发试验对性早熟性质具早期鉴 别诊断价值。 【关键词】 促性腺激素释放激素；青春期，早熟 Evaluation of simplified gonadotropin-releasing hormone analogue stimulation test for precocious puberty diagnosis MA Huamei，DU Minlian，CHEN Hongshan．13 Yanhong，sU zhe．Department of Pediatrics，The First Hospital Affiliated to Sun Yat—Sen University，G~angzhou 5 10080 【Abstract】 objective To assess the diagnostic value of the simplified gonadotropin—releasing hormone analogue(GnRHa)stimulation test for precocious puberty．Methods Seventy—four girls with signs 0f ad、ranced breast development received the simplified GnRHa stimulation test at the initial evaluation．They were divided into 4 groups，in which 42 girls with central precocious puberty(CPP，l9 in group CPP B2，23 in up CPP B3)，22 girls with premature thelarche(group PT)and 10 girls with peripheral precocious puberty(group PPP)．The 74 GnRHa stimulation tests were reviewed and analysed．Results In group PPP．no difference was found among the serum FSH or LH concentrations at 0，30 and 60 minutes during GnRH
