药品集中招标采购的利益协调机制研究 摘 要 药品集中招标采购制度是国家针对药品价格过高及药品采购中的腐败问题而推出的一项制度改革。然而,这一制度实施后,出现了诸多问题,成为广受关注的一个热点。本文运用制度经济学、信息经济学、福利经济学等理论,采用历史唯物主义、逻辑与历史相结合、定量和定性等分析方法,结合中国药品市场现状及特点,对药品采购制度展开理论与对策研究。 文章首先从制度变迁的规律着笔,描述了药品采购制度变迁的历史过程。继而对比分散采购制度的缺点,突出了招标采购制度的优点。指出药品采取集中招标采购是药品市场完善与发展的必经之路,肯定了集中招标采购的制度优势。以此为基础,对集中招标采购制度存在的问题进行分析,透过现象看本质,将问题的原因归结于药品采购主体利益的冲突。进而对药品采购主体的利益行为展开分析,指出利益主体冲突的激化造成了药品市场隐性契约环境和显性契约环境相冲突。这两种契约环境相“逆”的情况,使得药品市场上问题重重。文章最后在考虑到药品采购各主体、消费者和政府各方利益的基础上,建立了一个药品采购利益协调模型,并提出配套措施加以完善。 关键词:药品集中招标采购、制度分析、利益、显性契约、隐性契约 Abstract The Concentrated Bidding Purchases of Medicines System is a solution for declining the rising price of medicines. But a great deal of questions has appeared after implementing this method, which becomes the people’s care about and the government pays close attention to. This text uses theories such as institutional economics, information economics, and welfare economics and uses analytical methods such as historical materialism method, logical and historical method, quantitative and qualitative analyze method, combining with the current characteristic of Chinese medicines market to solve this problem Article writes from the rule of institutional innovation and describes the historical course of institutional innovation of medicine. Though comparing with the shortcoming of the Dispersing Methods of Drug Purchasing System to lighten the advantage of Concentrated Bidding Purchases of Medicines, pointing out it is the right way to develop medicines market and have great advantage. On the basis of this, it analyses the existed problem of the new system through the appearance to perceive the essence, ascribes the reason of the conflicts of the subjects’ interest in the medical purchase. This article also analyses the subjects’ behavior, points out why Recessive Contracts environment has problems with Dominance Contracts environment is because that subjects’ conflict existed.
