黑龙江省优化医疗资源配置的财政政策探讨

摘要

黑龙江省作为中国东北地区的重要省份，其医疗卫生服务体系的优化和医疗资源的合理配置对于提高居民健康水平、促进社会经济发展具有重要意义。近年来，随着国家对基本公共服务均等化的重视，黑龙江省在医疗卫生体制改革方面取得了一定进展，但仍面临着城乡和区域间医疗资源配置不均等、卫生机构资源配置不合理、公共卫生机构费用构成偏低、医疗机构卫生资源集中流向城市医院等问题。为了提高基本医疗卫生资源配置的均等化水平，促进基本公共服务均等化的发展，需要通过财政政策来解决上述问题。从公共产品理论，福利经济学理论以及资源优化配置理论入手，运用案例分析方法，阐述了黑龙江省医疗资源配置现状，分析了医疗资源配置中存在的问题，包括基本医疗卫生资源财政投入不均等、医疗资金筹集机制存在问题、财政对医疗基金的监管能力低等。在借鉴国内外优化医疗资源配置的财政政策经验的基础上，提出了优化医疗资源配置的财政政策建议，包括加强对基层医疗机构的财政投入、建立完善的公立医院财政保障机制、推进医疗资源信息化平台建设等。

Abstract

AsanimportantprovinceinNortheastChina,HeilongjiangProvinceisofgreatsignificanceforimprovingthehealthlevelofresidentsandpromotingsocialandeconomicdevelopmentthroughtheoptimizationofitsmedicalandhealthservicesystemandtherationalallocationofmedicalresources.Inrecentyears,withthecountrysemphasisontheequalizationofbasicpublicservices,HeilongjiangProvincehasmadesomeprogressinthereformofthemedicalandhealthsystem,butitstillfacestheproblemsofunequalallocationofmedicalresourcesbetweenurbanandruralareasandbetweenregions,unreasonableallocationofresourcesofhealthinstitutions,lowcostcompositionofpublichealthinstitutions,andconcentratedflowofhealthresourcesfrommedicalinstitutionstourbanhospitals.Inordertoimprovetheequalizationoftheallocationofbasicmedicalandhealthresourcesandpromotetheequalizationofbasicpublicservices,itisnecessarytosolvetheaboveproblemsthroughfiscalpolicy.Startingfromthetheoryofpublicgoods,thetheoryofwelfareeconomicsandthetheoryofoptimalallocationofresources,thispaperexpoundsthecurrentsituationofmedicalresourceallocationinHeilongjiangProvinceandanalyzestheproblemsexistingintheallocationofmedicalresources,includingtheunequalfinancialinvestmentinbasicmedicalandhealthresources,theproblemsinthemedicalfundraisingmechanism,andthelowabilityofthegove
