脑膜瘤的X刀治疗.DOCVIP

临床医学论文-脑膜瘤的X刀治疗 ????????????????????????? 作者：郝传国 周传芹 李春英 【关键词】? 脑膜瘤 ?【摘要】 目的 回顾性分析脑膜瘤X刀治疗后的临床及影像学变化。方法 1997年3月～2000年3月利用JX-100X刀系统治疗脑膜瘤40例。X刀治疗采用单次或分次照射，以80%或60%等剂量曲线覆盖病灶边缘，单次治疗病灶周边剂量12～24Gy，平均18.2Gy;分次治疗，一般分割2～3次，每次间隔1周，边缘总剂量18～26Gy，平均21.4Gy。全组40例均获12～48个月的随访，中位28个月。结果 40例患者治疗后均取得好疗效，治疗后6个月复查，临床症状或体征消失或缓解。在随访中有36例神经功能障碍减轻或消失，2例复发，症状加重，2例死亡;治疗后12、24、36个月CT或MRI复查25例症灶明显缩小或消失，11例瘤体无明显变化，2例较大脑膜瘤治疗后2～3年增大遂行第二次X刀治疗。结论 X刀治疗脑膜瘤能够达到治愈和控制肿瘤的目的，是脑膜瘤治疗的重要手段之一。 关键词 脑膜瘤 X刀 神经功能障碍 【Abstract】 Objective To analyze the imaginal and clinical changes of meningioma after the treatment with X-knife.Methods The JX-100X-Knife systemwas used to treat40cases suffering from meningioma，fromMarch1997to March2000.Eighty percent or sixty percent identical dose curve reached the verge of the tumor.Meningioma was treated by single time or fractioned times with X-Knife.The dose of the verge of the tumors which were treated once with X-knife was from12Gy to24Gy，theaverage dose18.2Gy.And the fractional treatment was divided into2or3times.The total dose of the verge of the tumors with fractional X-knife one time per two weeks was from18Gy to26Gy，the average dose21.4Gy.All40cases were followed up12～48months.The median was28months.Results The better effectwas performed after40cases were treated.The clinical symptoms or the clinical signs entirely alleviatˉed or disappeared.During the process of the following up，the neurological functional obstacle alleviated or disapˉpeared.The symptoms of2cases aggrevated and2cases died.Examined again by CT or MRI，meningioma of25cases were decreased evidently or disappeared after treatment12，24or36months，and11cases were no of change.The meningioma of2cases enlarged from2to3months after treatment，then were treated second time with X-knife.Conˉclusion Meningioma could be controlled or cured if meningioma was treated with X-knife.It is a kind of important treatment for meningioma.
