奎硫平联合氯丙嗪治疗急性精神分裂症对照研究_临床医学论文 奎硫平联合氯丙嗪治疗急性精神分裂症对照研究_临床医学论文 【摘要】 　　目的 探讨奎硫平联合小剂量氯丙嗪治疗精神分裂症患者兴奋症状的疗效与安全性。 方法 将60例精神分裂症患者随机分为两组各30例，两组均给予奎硫平治疗，研究组在此基础上联合氯丙嗪治疗。疗程1 mo。于治疗前及治疗第1 w、2 w、3 w、4 w末采用潘氏量表评定临床疗效，副反应量表评定不良反应。 结果 治疗4 w末，研究组显效率63.3%，总有效率90%；对照组分别为56.7%，93.3%。潘氏量表评分研究组治疗1 w末起、对照组治疗2 w末起较治疗前有极显著性下降（P均＜0.01）。研究组治疗1 w末、2 w末均较对照组下降显著（P均＜0.05）。两组不良反应均轻微，经过对症处理均可自行缓解或消失。 结论 奎硫平配合小剂量氯丙嗪控制精神分裂症患者的兴奋症状起效更快，且安全性高，依从性好；当患者的兴奋症状控制后可单用奎硫平治疗。 【关键词】 精神分裂症；奎硫平；氯丙嗪 　　A control study of quetiapine combined with chlorpromazine in the treatment of acute schizophrenia 　　【Abstract】 objective To explore efficacies and safety of quetiapine combined with small dose chlorpromazine in excitatory symptoms of schizophrenia. Methods 60 schizophrenics were randomly divided into research (n=30) receiving quetiapine combined with chlorpromazine and control group (n=30) doing single quetiapine for 1 month. The efficacies were assessed with The Positive and Negative Syndrome Scale(PANSS) and adverse effects the Treatment Emergent Symptom Scale(TESS) before treatment and at the end of the 1st，2nd，3rd and 4th week treatment. Results At the end of the 4th week，excellence and total effective rate was respectively 63.3% and 90% in the research group and 56.7% and 93.3% in the control group. Scores of the PANSS lowered very significantly since the end of the 1st week in the research and of the 2nd week in the control group compared with pretreatment(0.01)，those did more significantly in the research than in the control group at the end of the 1st and 2nd week(0.05). Adverse effects of both groups were light，and could relieve or disappear voluntarily via symptomatic treatment. Conclusion Quetiapine combined with small dose chlorpromaine takes effects faster and has higher safety and better conpliance in schizophrenia；quetiapine could be singly used afetr controlling excitatory symptoms. safer and much more effective than Quetiapine in the treatment of acute schizophrenia. 　　【Keywords】 Schizophrenia；quetiap
