河北医科大学第三医院10年1733例初次全髋关节置换病例回顾分析 　　摘要：目的 近年来行全髋关节置换病例逐渐增多，其病因组成、性别比例、年龄分布不断变化。本研究探讨患者的病因的组成、性别的比例、年龄的分布与关系以及全髋关节置换术在临床上逐年的发展变化。方法 统计分析我院2003年1月1日~2012年12月31日10年间初次行全髋关节置换术的病例资料，不包括髋关节翻修病例。对每年的病例数，患者的性别比例、年龄分布、病因变化进行比较。结果 共有1733例符合标准，男性845例，女性888例；致病原因：原发性股骨头缺血性坏死、股骨颈骨折、陈旧性股骨颈骨折及股骨颈骨折后股骨头缺血性坏死、髋臼发育不良、原发性髋骨关节炎、强直性脊柱炎、髋部骨肿瘤、类风湿性关节炎、髋臼骨折、转子间骨折、髋关节结核、先天性髋关节脱位，其中陈旧性股骨颈骨折及股骨颈骨折后股骨头缺血性坏死致病原因一致，将其统一归类为股骨颈骨折。患者年龄19~97岁，平均年龄58.34岁，男性55.07岁，女性61.46岁。结论 ①行全髋关节置换术的患者逐年增多；②女性比例高于男性；③患者平均年龄降低，女性平均年龄高于男性；④年轻患者比例增多；⑤主要病因：原发性股骨头缺血性坏死、股骨颈骨折； ⑥男性患者中原发性股骨头缺血性坏死为主，女性患者中股骨颈骨折为主；⑦股骨头缺血性坏死发病率逐年升高。 　　关键词：髋关节置换、性别比例、病因、年龄、股骨头缺血性坏死、股骨颈骨折 　　A Retrospective Analysis of 1733 Patients Undergoing Total Hip Replacement for the First Time for 10 Years in the Third Hospital of Hebei Medical University 　　YANG Xiang-Lei, YAN Zhi-kun,LI Hui-jie 　　(Department of orthopedics , The Third Hospital of Hebei Medical University,Research Institute of Hebei Province,Shijiazhuang 050051,Hebei,China) 　　Abstract：ObjectiveIn recent years, the number of patients undergoing total hip replacement is gradually increasing. The causes, gender and age of patients are changing. Our research is to find the difference, developments and changes between the 10 years.MethodsStatistical and analysis the patients undergoing total hip replacement for the first time from 2003.1.1 to 2012.12.31 without the revision cases. There are 11 pathogenies : avascular necrosis of the femoral head, femoral neck fracture, acetabular dysplasia, osteoarthritis, ankylosing spondylitis, bone tumor, rheumatoid, hip fracture,tuberculosis, congenital dislocation of the hip.
