肱骨髁上garlandⅲ型ⅱb型骨折夹板钢托外固定与克氏针内固定治疗74例临床观察.pdf

成都中医药大学硕士论文 摘 要 目的：观察比较74 例儿童肱骨髁上 gartlandⅢ型Ⅱb 型骨折闭合手法复位小 夹板钢托外固定与切开复位克氏针内固定两种方法的疗效，找出此类骨折较合适 的治疗方法。 方法：收集 2008 年 3 月至 2009 年 11 月间所收治的明显移位的儿童肱骨髁 上骨折病例共 74 例，按非随机同期对照分组法分为两组：A 组手法复位夹板外 固定组，B 组切开复位克氏针内固定组。将两组治疗后骨折复位后的 Baumann 角，骨折临床愈合时间、随访肘关节功能评价优良率、并发症的发生率及治疗费 用采用 SPSS8.0 统计学软件进行比较。 结果：对于明显移位的儿童肱骨髁上骨折骨折临床愈合时间、gartlandⅢ型 肘关节功能优良率、治疗费用方面 A 组优于 B 组（P ＜0.05 ）；gartland Ⅱb 型骨 折肘关节功能优良率 B 组优于 A 组（P ＜0.05 ）；复位后的Baumann 角度数，随 访肘关节功能评价总体优良率、并发症发生率 A 组与 B 组相比无统计学意义（P ＞0.05 ）。 结论：对于明显移位的儿童肱骨髁上骨折，闭合手法复位小夹板钢托外固定 与切开复位克氏针内固定两种方法均能取得满意疗效。闭合复位骨折愈合快、 gartlandⅢ型肘关节功能恢复较好且费用较低，是一种治疗儿童明显移位的肱骨 髁上骨折简、便、效、廉的方式，是此种骨折应当首先试用的方法。比较而言 gartland Ⅱb 型骨折手术治疗肘关节功能恢复较好。 关键词：儿童骨折 肱骨髁上骨折 夹板外固定 克氏针内固定 临床观察 1 成都中医药大学硕士论文 Abstract Objective: Observation and comparison of children supracondylar gartland Ⅲ type Ⅱb type fracture closed manipulative reduction and splint steel prop open reduction and Kirschner wire fixation and internal fixation efficacy of two methods to find the best treatment Methods: Collected from March 2008 to November 2009 among the children admitted to the apparent shift-type supracondylar fractures were 74 cases of Non- randomized concurrent control groups were divided into A: manipulative reduction and external fixation group and B group: Open reduction and Kirschner wire fixation.
