HPV DNA检测在宫颈病变病人分层管理中的应用 　　【摘要】目的： 探讨不同HPV感染型别、不同年龄等对宫颈病变发展程度的影响，用于指导病人分层管理的合理策略。方法： 采用凯普医用核酸分子快速导流杂交基因芯片技术，对2012年4月至2013年3月就诊于我院妇产科的1070名女性进行生殖道21种人乳头瘤病毒（HPV）分型检测。结果： HPV总感染率17.20%，常见感染型别依次为HPV16（27.17%），HPV58（21.20%），HPV31（11.96%），HPV52（9.24%）及HPV18（7.07%）。20～25岁和55～59岁两个年龄段，HPV阳性率最高，但40～44岁HPV16、33、31三种高危型阳性率最高。35～39岁CINⅡ及以上病变的比率最高。结论： 不同情况的HPV阳性病人需分层管理，区别对待。 　　【关键词】人乳头瘤病毒；基因型别；年龄；宫颈上皮内瘤变 　　Application of HPV DNA detection in hierarchical management of patientsSU Li， LIU Haiqing， XI Lijun， REN Jing， FENG Liyan， YING Dan. Methods： Specimens from 1070 women who visited the Longnan Hospital from April 2012 to March 2013 were tested for HPV genotypes by flow-through hybridization and gene chip technology. Results： The infection rate of HPV was 17.20%. The most common HPV genotypes were HPV16 （27.17%）， HPV58 （21.20%）， HPV31 （11.96%）， HPV52 （9.24%） and HPV18 （7.07%）. The age groups with the highest positive rate were group 20～25 and group 55～59. But group 40～44 had the highest positive rate of high risk type of HPV 16， 33 and 31. The grade of cervical lesions in group 35～39 was the highest. Conclusions： It is necessary to differentiate different HPV-positive patients. 　　【Key words】Human papilloma virus； Genotype； Age； Cervical intraepithelial neoplasias （ CIN） 　　【中图分类号】R737【文献标志码】A 　　宫颈癌作为威胁女性健康的第二大恶性肿瘤，近年来其死亡率虽然有所下降，但发病率仍持续增长[1]，且宫颈癌对于发展中国家的威胁尤甚。Ahmedin等[2]的研究显示，在所有威胁女性健康的恶性肿瘤中，发达国家宫颈癌的发病率和死亡率已跌至第9位和第10位，而在发展中国家发病率和死亡率仍居高不下，均排在第2位。由此可以看出发展中国家的宫颈癌早期诊治仍是任重而道远。此外，随着女性宫颈癌筛查力度的增加，越来越多的关注集中到了宫颈癌筛查上。本研究通过分析1070例患者HPV DNA分型检测及病理诊断结果，探讨不同年龄、不同HPV感染型别等对宫颈病变发展程度的影响，用于指导病人分层管理的合理策略。 　　1材料和方法 　　1.1一般资料 　　2012年4月至2013年3月在我院门诊就诊、自愿接受HPV基因分型检测的有性生活的女性1070名，年龄在20～72岁，平均年龄（40.87±8.84）
