缺血性脑卒中患者早期神经功能恶化的相关危险因素分析 　　[摘要] 目的 探讨缺血性脑卒中患者早期神经功能恶化的相关危险因素。 方法 选取本院2013年4月～2015年1月收治的80例缺血性脑卒中患者作为研究对象，按照是否发生早期神经功能恶化分为早期神经功能恶化组和非早期神经功能恶化组。采用单因素方差分析和多因素Logistic回归分析比较两组的一般资料和各项临床指标，总结发生早期神经功能恶化的相关因素。 结果 在80例缺血性脑卒中患者中，25例（31.3%）发生早期神经功能恶化。多因素Logistic回归分析显示，NIHSS评分高、颈动脉狭窄≥50%、心房颤动、糖尿病以及高血压都是发生早期神经功能恶化的危险因素（P0.05）。 结论 缺血性脑卒中患者发生早期神经功能恶化的危险因素较多，应针对性给予干预措施以改善预后。 　　[关键词] 缺血性脑卒中；早期神经功能恶化；影响因素 　　[中图分类号] R743.3 [文献标识码] A [文章编号] 1674-4721（2015）09（a）-0051-03 　　[Abstract] Objective To explore the relevant risk factors of early neurological deterioration in patients with ischemic cerebral apoplexy. Methods 80 patients with ischemic cerebral apoplexy in our hospital from April 2013 to January 2015 were selected and divided into the group of early neurological deterioration and the group of non-early neurological deterioration according to whether early neurological deterioration occurred.Single-factor ANOVA and multi-factor Logistic regression analysis were applied to compare the general data and each clinical index between two groups，and relevant factors for incidence of early neurological deterioration was summarized. Results In 80 patients with ischemic stroke，25 patients （31.3%） had early neurological deterioration.Multi-factor Logistic regression analysis detected that high score of NIHSS，carotid artery stenosis ≥50%，ventricular atrial fibrillation，diabetes and hypertension were all risk factors for early neurological deterioration （P0.05）. Conclusion Risk factors for early neurological deterioration in patients with ischemic cerebral apoplexy are relatively various，and specific intervention measures should be applied so as to improve patients′prognosis. 　　[Key words] Ischemic cerebral apoplexy；Early neurological deterioration；Influencing factor 　　早期神经功能恶化是缺血性脑卒中患者较为多见的病症，又被称为进展性脑卒中。研究显示，缺血性脑卒中发病后一段时间内，神经症状仍会进一步加重，进而导致致死、致残率大幅度增加[1]。此外，脑卒中患者发生该现象的概率高达24%～39%，因此早期发现并给予及时有效的治疗措施对脑卒中患者有着重要意义。本研究选取本院的缺血性脑卒中患者作为研究对象，旨在探讨影响缺血性脑卒中患者发生早期神经功能恶化的影响因素，为临床诊治工作提供理论依据。 　　1
