主动脉外科手术中的血液保护 　　摘要 目的 总结主动脉外科，特别是夹层外科手术中应用多种血液保护措施的经验，达到少用血或不用血的目的。 　　方法 回顾分析我院胸心外科2013年1月至2014年6月收治的128例主动脉病变患者特别是主动脉夹层外科手术中的用血及血液保护情况。综合的血液保护措施有：1）术前酌情经颈静脉放血400～800ml;2）术中在每个吻合口使用Coseal胶水封闭;3）常规使用自体血回输装置;4）吻合完毕后常规用残留的主动脉外膜或牛心包补片做主动脉包埋及右心房内引流;5）常规使用改良超滤;6）常规使用凝血酶原复合物及纤维蛋白原;7）上述措施无效，难以控制的非外科性出血时，使用诺其（第 Ⅷ 因子）。 　　结果 27例中度低温体外循环主动脉手术（Bentall术、单纯升主动脉置换术、半弓人工血管置换术）有13例未输血，101例深低温停循环主动脉手术（主动脉全弓置换术），采用综合的血液保护措施后，红细胞、血浆、冷沉淀及血小板用量明显减少。 　　结论 通过及时正确的多种综合血液保护措施，主动脉外科手术少输血甚至不输血是完全可能的。 　　关键词 主动脉夹层;血液保护;体外循环;深低温停循环 　　通讯作者：潘友民，email：panyoumin@126.com 　　Blood conservation in aortic dissection surgery 　　Zhou Manling，Wang Yuan，Wan Lili，Li Jun，Wei Xiang，Pan Youmin 　　Department of Cardiothoracic Surgery，Tongji Hospital，Tongji Medical College， Huazhong University of Science and Technology，Wuhan 430030，China） 　　Abstract： Objective To summarize the experiences of a variety of blood conservation used in the aortic dissection surgery， to aim at less or no blood transfusion. 　　Methods A retrospective analysis of the operative blood usage and conservation was done in 128 patients with aortic lesions especially aortic dissection surgery from January 2013 to June 2013 in Tongji Hospital.
