Appendix A – Services to be Provided by Grantee Northern California Presbyterian Homes and Services Services Connection Program July 28, 2014 – June 30, 2017 I. Purpose The purpose of this grant is to link seniors and adults with disabilities living in public housing with available services provided in the community. The goal of this program is to enhance residents’ abilities to age in place, avoid premature institutionalization, and build community in their environments. II. Definitions Grantee Northern California Presbyterian Homes and Services (NCPHS) CARBON Human Service Agency’s Contracts Administration Reporting and Billing Online (CARBON) system DAAS San Francisco Department of Aging and Adult Services City City and County of San Francisco, a municipal corporation. SFHA San Francisco Housing Authority Adult with disability Person 18 - 59 years of age or older living with a disability. ADL Activities of Daily Living IADL Instrumental Activities of Daily Life Disability A condition attributable to mental or physical impairment, or a combination of mental and physical impairments including hearing and visual impairments, that results in substantial functional limitations in one or more of the following areas of major life activity: self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, economic self-sufficiency, cognitive functioning, and emotional adjustment Senior Person who is 60 years or older III. Target Population Individuals 60 years of age or older (and/or) individuals between 18 and 59 of age who are living with disabilities. Services must target clients who are members of one or more of the following target groups that have been identified as demonstrating the greatest economic and social need.
