奥曲肽联合云南白药治疗急性上消化道出血45例临床观察 　　doi：10.3969/j.issn.1007-614x.2014.20.53 　　摘 要 目的：探讨奥曲肽联合云南白药治疗急性上消化道出血的临床疗效。方法：2011年4月-2013年12月收治急性上消化道出血患者90例，随机分为治疗组和对照组，各45例。对照组给予泮托拉唑40 mg静滴，治疗组在对照组的基础上加用奥曲肽注射液0.1 mg缓慢静脉推注，继之以25 μg/h持续静滴，同时口服云南白药1.5 g，3次/日。观察两组治疗效果。结果：治疗组总有效率91.11%，对照组总有效率77.78%，治疗组总有效率明显高于对照组，差异具有统计学意义（P0.05）。结论：奥曲肽联合云南白药治疗急性上消化道出血疗效显著，值得推广。 　　关键词 奥曲肽 云南白药 急性上消化道出血 临床观察 　　Clinical observation of octreotide combined with Yunnan Baiyao in the treatment of 45 cases of acute upper gastrointestinal bleeding 　　Hua Yan，Wang Huaguang，Tang Yong 　　The Peoples Hospital of Yingshang County，Fuyang City，Anhui 236200 　　Abstract Objective：To explore the clinical effect of octreotide combined with Yunnan Baiyao in the treatment of acute upper gastrointestinal bleeding.Methods：90 cases with acute upper gastrointestinal bleeding were selected from April 2011 to December 2013.They were randomly divided into the treatment group and the control group with 45 cases in each.The control group were given static drop of 40 mg pantoprazole.On the basis of the control group，the treatment group were given slow intravenous injection of 0.1 mg octreotide injection，followed by 25 μg/hour continuous infusion，at the same time，oral 1.5 g Yunnan Baiyao，3 times/day.The treatment effects of the two groups were observed.Results：The total effective rate of the treatment group was 91.11%.The total effective rate of the control group was 77.78%.The total effective rate of the treatment group was significantly higher than the control group.The difference was statistically significant（P0.05）.Conclusion：Octreotide combined with Yunnan Baiyao in the treatment of acute upper gastrointestinal bleeding has significant effect.It is worthy of promotion. 　　Key words Octreotide；Yunnan Baiyao；acute upper gastrointestinal bleeding；Clinical observation 　　上消化道出血是常见的急症之一，主要临床表现为呕血和黑粪，往往伴有血容量减少引起的急性周围循环衰竭，其起病急、病情进展快、病死率较高，应该引起临床医生的广泛重视[1]。其治疗原则是积极控制出血、治疗原发病，必要时输血及手术治疗。2009年4月-2013年12月我院使用奥曲肽联合云南白药治疗急性上消化道出血，取得了
