Methods 37 cases of summary clinical data of complex proximal humeral fractures. According to Neer classification of which 11 cases of four-part fractures associated with humeral head crushed by using artificial humeral head prosthesis replacement surgery, and conduct early shoulder function exercises. 11 patients were over 60 years of age, of which 6 cases with shoulder dislocation (2 cases associated with brachial plexus injury). Results 11 patients were followed up for 6 to 12 months, an average of 10 months. Constant Functional Score overall average score 83, the overall excellent rate of 82%. No one cases of re-dislocation of joints occurred, no operative complications, patients were satisfied with the subjective. Conclusions Elderly complex proximal humeral fractures humeral head replacement surgery, rotator cuff repair surgery effectively and size of nodules, combined with a reasonable joint function of early exercise can achieve a satisfactory therapeutic effect.
