Objective.Highlightingtheapelinsystemwouldpresentanewtherapeutictargetforliverdisease.Apelin;endogenousligandfor theorphanreceptorAPJ,wasrecentlysuggestedtobeassociatedwith?brosisprogressionandcirrhosisinadditiontoinsulin resistance(IR)andin?ammation.Thepresentstudywasconducted toevaluatebloodapelinlevelchangesamong73chronic hepatitisC(CHC)Egyptianpatientsandifassociatedwithbodymassindex(BMI),IR,andtumornecrosisfactor-alpha(TNF- α).Serumapelinlevelsweresigni?cantlyhigherinpatientswithCHCwithmedianvalue(3.25)whencomparedwithcontrols (1.11),atP0.0001,withsigni?cantapelinvariationsamongasymptomaticcarriers(ASC),?brosis,andcirrhosispatients,and alsoamongobeseandnonobesepatients.MultipleregressionanalysisdepictedthatBMI,triglycerides,andtotalcholesterolwere independentcorrelationfactorstoapelinlevels,whereasTNF-αwasfoundtobesigni?cantlynegativelycorrelatedtoadjusted apelininCHCpatients(r = ?0.5944,P 0.0001).IRwaspositivelycorrelatedtoadjustedapelininCHCpatients(r =0.2663, P0.05).Conclusion.ApelinlevelvariesamongstagesofCHC,whichmaycontributeto?brosisprogression.Inaddition,obesity andIRcouldactascomorbidfacto
