The aim of this paper is to describe a fatal case of hemophagocytic lymphohistiocytosis (HLH) in a patient with severeheartfailure,whowastreatedwithlow-dosepropranolol.PatientandInterventions.Wereportona7-month-oldboywith Downssyndromewhowasbornwithanunbalanced,leftdominantatrioventricularseptaldefectandaorticcoarctation.Despite coarctationrepairandpulmonaryarterybandinghedevelopedintractableheartfailureandfeverofunknownorigin.Sincehe remainedinheartfailurehereceivedatrialoflow-dosepropranololtostabilizehiscardiopulmonarystatus,whichresultedin unexpectedimmunomodulatorye?ects.MeasurementsandMainResult.Immunoactivationwasevidencedbyhighconcentrations ofprocalcitonin,solubleCD25,tumornecrosisfactorα,andinterleukin6and8.Propranololresultinginhepaticcompromise asindicatedbyhighlactatedehydrogenaseandalanineaminotransferaselevels.Atherapeuticswitchfrompropranololtothe β1-receptorblockermetoprol
