Background.Asthmaandobesityarecommon;howevertheimpactofobesityuponasthmaremainsuncertain.Objectives .To assessrelationshipsbetweenobesityandfatmasswithairwayin?ammation,lungfunction,anddiseasecontrolinpatientswith refractoryasthma.Methods.151refractoryasthmapatientswerecharacterisedformeasuresofairwayin?ammation,lungfunction, Juniperasthmacontrolquestionnaire(JACQ),bodymassindex(BMI),andfatmassindex(FMI)derivedfromdualenergyX-ray absorptiometry.Patientswerereassessedover12months.Results.74%ofpatientshadanelevatedBMI.BMIandFMIcorrelated (r =0.9,P.001).FMIandJACQcorrelatedinmen(r =0.3,P =.01).After12months23%lostweight.Weightchangeover 12monthscorrelatedwithFEV1change(r=?0.3,P=.03),butnotwithchangeinJACQorexacerbations.Conclusion .Increased fatmassiscommoninrefractoryasthmaandisassociatedwithasthmasymptomcontrolinmen.Lossofweightisassociatedwith improvementinlungfunctioninrefractoryasthma. 1.Introduction Unsurprisingly these two common conditions often coexist, but whether obesity is a major determinant of asthma severity or whether weight reduction impacts on asthma control or exacerbations remains uncertain.
