Wedescribetheclinical,grossandmicroscopicfeaturesofundi?erentiateduterinestromalsarcomaassociatedwithosteoclast- likegiantcells.Acaseoflow-gradeendometrialstromalsarcomaisalreadydescribedinassociationwithosteoclast-likegiant cells; however,thecurrent casedi?ers inthatthetumorwasahighgrade anddidnotshowanyevidenceofsmoothmuscle orepithelioiddi?erentiationandwasshowntobestronglypositiveforCD10andfocallyforWT-1andInhibinsupportingan endometrialstromalorigin.Theassociatedosteoclast-likegiantcellswereabundant,evenlydistributed withinthetumorand showed strong positivity for CD68. Interestingly, rare (less than 2%) giant cells also showed weak cytoplasmic positivity for b-hCG.Thetumorin?ltrateddeepintothemyometriumandhadmarkedlymphovascularinvasion.Althoughtheregionallymph nodesandperitonealwashingswerenegative,thelesionshowedahighlyaggressiveclinicalcourse.Despitetreatment,thetumor disseminatedwithintheabdominalcavityandlungsandultimatelyledtothepatient’sdemisewithin9monthsofthediagnosis. 1.Introduction whichmaybethesourceofdiagnosticdi?culties.Probably thevery?rstcaseofpoorlydi?erentiatedendometrialstro- malsarcomawithosteochondromatousdi?erentiationand benign appearing giant cells is documented approximately 30yearsagobyEvansin1982[3]howeverthisneoplasmwas notdescrib
