Cardiovascular disease (CVD) mortality varies across racial and ethnic groups in the U.S., and the extent that knownriskfactorscanexplainthedifferenceshasnotbeenextensivelyexplored.Methods.Weexaminedtheriskofdying fromacutemyocardialinfarction(AMI)andotherheartdisease(OHD)among139,406African-American(AA),NativeHawaiian (NH), Japanese-American (JA), Latino and White men and women initially free from cardiovascular disease followed prospectivelybetween1993–1996and2003intheMultiethnicCohortStudy(MEC).Duringthisperiod,946deathsfromAMI and2,323deathsfromOHDwereobserved.RelativerisksofAMIandOHDmortalitywerecalculatedaccountingforestablished CVDriskfactors:bodymassindex(BMI),hypertension,diabetes,smoking,alcoholconsumption,amountofvigorousphysical activity, educational level, diet and, for women, type and age at menopause and hormone replacement therapy (HRT) use. Results. EstablishedCVDriskfactorsexplainedmuchoftheobservedracialandethnicdifferencesinriskofAMIandOHD mortality.Afteradjustment,NHmenandwomenhadgreaterrisksofOHDthanWhites(69%excess,P,0.001and62%excess, P=0.003,respectively),andAAwomenhadgreaterrisksofAMI(48%excess,P=0.01)andOHD(35%excess,P=0.007).JAmen hadlowerrisksofAMI(51%deficit,P,0.001)andOHD(27%deficit,P=0.001),asdidJAwomen(AMI,37%deficit,P=0.03; OHD,40%deficit,P=0.001).LatinoshadunderlyinglowerriskofAMIdeath(26%deficitinmenand35%inwomen,P=0.03).
