However,itsvalueinpatientswithcomaaftercardiacarrest(CA),particularlyinthesettingoftherapeutic hypothermia(TH),isonlybeginningtobeelucidated.TheaimofthisstudywastoexaminewhethercEEG performedduringTHmaypredictoutcome. Methods:FromApril2009toApril2010,weprospectivelystudied34consecutivecomatosepatientstreatedwith THafterCAwhoweremonitoredwithcEEG,initiatedduringhypothermiaandmaintainedafterrewarming.EEG backgroundreactivitytopainfulstimulationwastested.WeanalyzedtheassociationbetweencEEGfindingsand neurologicoutcome,assessedat2monthswiththeGlasgow-PittsburghCerebralPerformanceCategories(CPC). Results:ContinuousEEGrecordingwasstarted12±6hoursafterCAandlasted30±11hours.NonreactivecEEG background(12of15(75%)amongnonsurvivorsversusnoneof19(0)survivors;P0.001)andprolonged discontinuous“burst-suppression”activity(11of15(73%)versusnoneof19;P0.001)weresignificantly associatedwithmortality.EEGseizureswithabsentbackgroundreactivityalsodifferedsignificantly(sevenof15 (47%)versusnoneof12(0);P=0.001).Inpatientswithnonreactivebackgroundorseizures/epileptiformdischarges oncEEG,noimprovementwasseenafterTH.NonreactivecEEGbackgroundduringTHhadapositivepredictive valueof100%(95%confidenceinterval(CI),74to100%)andafalse-positiverateof0(95%CI,0to18%)for mortality.AllsurvivorshadcEEGbackgroundreactivity,andthemajorityofthem(14(74%)of19)hadafavorable outcome(CPC1or2). Conclusions:ContinuousEEGmonitoringshowinganonreactiveordiscontinuousbackgroundduringTHis stronglyassociatedwithunfavorableoutcomeinpatientswithcomaafterCA.Thesedatawarrantlargerstudiesto confirmthevalueofcontinuousEEGmonitoringinpredictingprognosisafterCAandTH.
