Abbate-Dagaetal.BMCPsychiatry2013,13:14 /1471-244X/13/14 RESEARCH ARTICLE OpenAccess Psychosomaticsyndromesandanorexianervosa GiovanniAbbate-Daga*,NadiaDelsedime,BarbaraNicotra,CristinaGiovannone,EnricaMarzola, FedericoAmiantoandSecondoFassino Abstract Background:Inspiteoftheroleofsomepsychosomaticfactorsasalexithymia,moodintolerance,and somatizationinbothpathogenesisandmaintenanceofanorexianervosa(AN),fewstudieshaveinvestigatedthe prevalenceofpsychosomaticsyndromesinAN.TheaimofthisstudywastousetheDiagnosticCriteriafor PsychosomaticResearch(DCPR)toassesspsychosomaticsyndromesinANandtoevaluateifpsychosomatic syndromescouldidentifysubgroupsofANpatients. Methods:108ANinpatients(76ANrestrictingsubtype,AN-R,and32ANbinge-purgingsubtype,AN-BP)were consecutivelyrecruitedandpsychosomaticsyndromeswerediagnosedwiththeStructuredInterviewforDCPR. Participantswereaskedtocompletepsychometrictests:BodyShapeQuestionnaire,BeckDepressionInventory, EatingDisorderInventory–2,andTemperamentandCharacterInventory.Dataweresubmittedtoclusteranalysis. Results:Illnessdenial(63%)andalexithymia(54.6%)resultedtobethemostcommonsyndromesinoursample. Clusteranalysisidentifiedthreegroups:moderatepsychosomaticgroup(49%),somatizationgroup(26%),and severepsychosomaticgroup(25%).ThefirstgroupwasmainlyrepresentedbyAN-Rpatientsreportingoftenonly illnessdenialandalexithymiaasDCPRsyndromes.Thesecondgroupshowedmoresevereeatinganddepressive symptomatologyandfrequentlyDCPRsyndromesofthesomatizationcluster.ThanatophobiaDCPRsyndromewas alsorepresentedinthisgroup.ThethirdgroupreportedlongerdurationofillnessandDCPRsyndromeswere highlyrepresented;inparticular,allpatientswerefoundtoshowthealexithymiaDCPRsyndrome. Conclusions:TheseresultshighlighttheneedofadeepassessmentofpsychosomaticsyndromesinAN. Psychosomaticsyndromescorrelateddifferentlywithbothseverityofeatingsymptomatologyanddurationof illness:therefore,DCPRcouldbeeffectivetoachievetailoredtreatments.
