Linetal.DiagnosticPathology2012,7:174 /content/7/1/174 CASE REPORT OpenAccess Pulmonarysclerosinghemangiomapresenting withdensespindlestromacells:apotential diagnosticpitfall Xu-YongLin1,2,YanWang1,2,Chui-FengFan1,2,YangLiu1,2,Juan-HanYu1,2,Shun-DongDai1,2,LiangWang1,2 andEn-HuaWang1,2* Abstract Pulmonarysclerosinghemangioma(PSH)isanuncommonpulmonarytumor.Histologically,PSHtypicallyconsists oftwotypesofcells,surfacecuboidalcellsandpolygonalcells,fourarchitecturalpatternsincludingpapillary, sclerotic,solid,andhemorrhagic.Herein,wepresentacaseofPSHina59-year-oldChinesefemale.Thetumorwas predominantlycomposedofsolidareapresentingwithdiffusespindlecellsratherthanpolygonalcells.Focally, classicalpapillaryandscleroticareacouldbeseen.Immunohistochemicalstainingshowedthatthespindlecells werepositiveforTTF-1,EMA,Actin(SM)andVimentin,andnegativeforcytokeratin,cytokeratin7,cytokeratin5/6, surfactantapoproteinA,surfactantapoproteinB,CD34,CD99,S-100,HMB45,Desmin,Synaptophysin,CD56,ALK andCalretinin.Theimmunophenotypeofthedensespindlecellsinthiscasewassimilartothatofthepolygonal cells,andthusthespindlecellsmaybethevariantsofpolygonalcells.Basedonmorphologicfeaturesandthe immunohistochemicalprofile,thetumorwasdiagnosedasaPSH.Thesignificanceofspindlecellschangeis unclearforus.Toourknowledge,thisisthefirstreportedcaseofPSHshowingdensespindlecellsinsolidarea. Thiscaserepresentsapotentialdiagnosticpitfall,asitmaybemisdiagnosedasamesenchymaltumorsuchas inflammatorymyofibroblastictumor,synovialsarcoma,solitaryfibroustumor,leiomyoma,orevenmesothelioma, especiallyifthespecimenislimitedorfromfine-needleaspiration.
