Objective:TodeterminetheassociationbetweenlungfunctionandAHRandquantitativeatopyinalargesample ofadultsfromtheUK. Methods:FEV1andFVC(%predicted)weremeasuredusingspirometryandairwayresponsivenessby methacholinechallenge(5-breathdosimeterprotocol)in983subjects(randomsampleof800parentsofchildren enrolledinapopulation-basedbirthcohortenrichedwith183patientswithphysician-diagnosedasthma).Atopic statuswasassessedbyskinpricktests(SPT)andmeasurementofsIgE(commoninhalantallergens).Wealso measuredindoorallergenexposureinsubjects’homes. Results:Spirometrywascompletedby792subjectsand626underwentmethacholinechallenge,with100(16.0%) havingAHR(dose-responseslope25).UsingsIgEasacontinuousvariableinamultiplelinearregressionanalysis, wefoundthatincreasinglevelsofsIgEtomite,catanddogweresignificantlyassociatedwithlowerFEV1(mitep -8 =0.001,catp=0.0001,dogp=2.95×10 ).Similarfindingswereobservedwhenusingthesizeofwhealonskin testingasacontinuousvariable,withsignificantlypoorerlungfunctionwithincreasingskintestsize(mitep=8.23 ×10-8,catp=3.93×10-10,dogp=3.03×10-15,grassp=2.95×10-9).Theassociationbetweenquantitative atopywithlungfunctionandAHRremainedunchangedwhenwerepeatedtheanalysesamongstsubjectsdefined assensitisedusingstandarddefinitions(sIgE0.35kUa/l,SPT-3mmnegativecontrol).
