Lossiusetal.CriticalCare2011,15:R26 /content/15/1/R26 RESEARCH OpenAccess Revisitingthevalueofpre-hospitaltracheal intubation:analltimesystematicliteraturereview extractingtheUtsteinairwaycorevariables HansMortenLossius1,2*,StephenJMSollid1,2,3,MariusRehn1,4,5,DavidJLockey6,7 Abstract Introduction:Althoughtrachealintubation(TI)inthepre-hospitalsettingisregularlycarriedoutbyemergency medicalservice(EMS)providersthroughouttheworld,itsvalueiswidelydebated.Heterogeneityinprocedures, providers,patients,systemsandstatedoutcomes,andinconsistencyindatareportingmakescientificreports difficulttointerpretandcompare,andthemajorityareoflimitedquality.Tohuntdownwhatisreallyknown aboutthevalueofpre-hospitalTI,wedeterminedtherateofreportedUtsteinairwayvariables(28corevariables and12fixed-systemvariables)foundincurrentscientificpublicationsonpre-hospitalTI. Methods:WeperformedanalltimesystematicsearchaccordingtothePRISMAguidelinesofMedlineand EMBASEtoidentifyoriginalresearchpertainingtopre-hospitalTIinadultpatients. Results:From1,076identifiedrecords,73originalpaperswereselected.Informationwasextractedaccordingtoan Utsteintemplatefordatareportingfromin-the-fieldadvancedairwaymanagement.Fifty-ninestudieswerefrom NorthAmericanEMSsystems.Ofthese,46(78%)describedservicesinwhichnon-physiciansconductedTI.In12of the13non-NorthAmericanEMSsystems,physiciansperformedthepre-hospitalTI.Overall,twowererandomised controlledtrials(RCTs),and65wereobservationalstudies.Noneofthestudiespresentedthecompletesetof recommendedUtsteinairwayvariables.Themediannumberofcorevariablesreportedwas10(max21,min2,IQR 8-12),andthemediannumberoffixedsystemvariableswas5(max11,min0,IQR4-8).Amongthemost frequentlyreportedvariableswere“patientcategory”and“servicemissiontype”,reportedin86%and71%ofthe studies,respectively.Amongtheleast-reportedvariableswere“co-morbidity”and“typeofavailableventilator” ,both reportedin2%and1%ofthestudies,respectively.
