IdentifyingpatientsathighriskofdevelopingAOFandtargetingthemwithpreventativestrategiesmaybe effective.Ourstudyquestionwas:inapopulationofICUpatientsreceivingpositivepressurerespiratorysupport (invasiveornon-invasive)intheabsenceofnon-respiratoryAOF,whatisthe14-dayincidenceof,riskfactorsfor andtimetoacuteorganfailure? Methods:Inaninternationalprospectivecohortstudy,patientsreceivingpositivepressurerespiratorysupport (invasiveornon-invasive)intheabsenceofnon-respiratoryAOFwereenrolledandfollowedfor14days.The primaryoutcomemeasurewastheincidenceofanyAOF(definedasSOFA3to4)duringfollow-up. Results:Atotalof123of766screenedpatients(16.1%)wereenrolled.Dataarereportedfor121patients.Intotal, 45outof121patients(37.2%)developedAOF.MortalityrateswerehigherinthosewithAOF:17.8%versus4.0% OR5.11,P=0.019)forICUmortality;and28.9%versus11.8%(OR2.80,P=0.019)forhospitalmortality.MedianICU lengthofstaywasalsolongerinthosewithAOF(11versus3.0days;P0.0001).Hypoxemicrespiratoryfailure (P=0.001)andcardiovasculardysfunction(thatis,SOFA1to2;P=0.03)wereassociatedwithAOF.Themedian timetofirstAOFwastwodays. Conclusions:Patientsreceivingpositive(invasiveornon-invasive)pressurerespiratorysupportintheabsenceof non-respiratoryAOFarecommonlyadmittedtoICU;AOFisfrequentinthesepatients.Organfailuredeveloped withinashortperiodafteradmission.Hypoxemicrespiratoryfailureandcardiovasculardysfunctionwerestrongly associatedwithAOF.
