Even though the economic evaluation of interventionstoreducethisburdenisessentialtoguidehealthpolicies,dataoncostsassociatedwithLBWarescarce.This studyaimstoestimatethecoststothehealthsystemandtothehouseholdandtheDisabilityAdjustedLifeYears(DALYs) arisingfrominfantdeathsassociatedwithLBWinSouthernMozambique. Methods and Findings: Costsincurredbythehouseholdswerecollectedthroughexitsurveys.Healthsystemcostswere gatheredfromdataobtainedonsiteandfrompublishedinformation.DALYsduetodeathofLBWbabieswerebasedonlocal estimatesofprevalenceofLBW(12%),verylowbirthweight(VLBW)(1%)andofcasefatalityratescomparedtonon-LBW weight babies [for LBW (12%) and VLBW (80%)]. Costs associated with LBW excess morbidity were calculated on the incrementalnumberofhospitaladmissionsinLBWbabiescomparedtonon-LBWweightbabies.Directandindirecthousehold costsforroutinehealthcarewere24.12US$(CI95%21.51;26.26).Anincreaseinbirthweightof100gramswouldleadtoa 53%decreaseinthesecosts.Directandindirecthouseholdcostsforhospitaladmissionswere8.50US$(CI95%6.33;10.72).Of the3,322livebirthsthatoccurredinoneyearinthestudyarea,healthsystemcostsassociatedtoLBW(routinehealthcareand excessmorbidity)andDALYswere169,957.61US$(CI95%144,900.00;195,500.00)and2,746.06,respectively. Conclusions:ThisfirstcostevaluationofLBWinalow-incomecountryshowsthatreducingtheprevalenceofLBWwould translate
