MethodsandFindings:Wefollowed69,269womenaged42–67inNurses’HealthStudyI(NHSI,1988–2008),and107,915 women aged 25–42 in NHS II (1989–2007) without diabetes, cardiovascular disease, and cancer at baseline. Participants wereaskedhowlongtheyhadworkedrotatingnightshifts(definedasatleastthreenights/monthinadditiontodaysand eveningsinthatmonth)atbaseline.Thisinformationwasupdatedevery2–4yearsinNHSII.Self-reportedtype2diabetes wasconfirmedbyavalidatedsupplementaryquestionnaire.Wedocumented6,165(NHSI)and3,961(NHSII)incidenttype 2 diabetes cases during the 18–20 years of follow-up. In the Cox proportional models adjusted for diabetes risk factors, durationofshiftworkwasmonotonicallyassociatedwithanincreasedriskoftype2diabetesinbothcohorts.Compared withwomenwhoreportednoshiftwork,thepooledhazardratios(95%confidenceintervals)forparticipantswith1–2,3–9, 10–19,and$20yearsofshiftworkwere1.05(1.00–1.11),1.20(1.14–1.26),1.40(1.30–1.51),and1.58(1.43–1.74,p-valuefor trend,0.001),respectively.Furtheradjustmentforupdatedbodymassindexattenuatedtheassociation,andthepooled hazardratioswere1.03(0.98–1.08),1.06(1.01–1.11),1.10(1.02–1.18),and1.24(1.13–1.37,p-valuefortrend,0.001).
