Data on PCV7-induced mucosal antibodies in relation to systemic or natural anticapsular antibodiesarescarce. Methods:Inarandomizedcontrolledsetting,childrenreceivedPCV7atage2and4months(2-dosegroup),atage2,4and 11months(2+1-dosegroup)ornoPCV7(controlgroup).From188childrenpairedsalivasampleswerecollectedat12and 24monthsofage.Fromasubgroupof15immunizedchildrenalsoserumsampleswerecollected.IgGandIgAantibody- levelsweremeasuredbymultipleximmunoassay. Results:At12months,bothvaccinegroupsshowedhigherserumandsalivaIgG-levelsagainstvaccineserotypescompared withcontrolswhichsustaineduntil24monthsformostserotypes.SalivaryIgG-levelswere10–20-foldlowercomparedto serumIgG,however,serumandsalivaIgG-levelswerehighlycorrelated.SerumandsalivaryIgA-levelswerehigherinboth vaccinegroupsat12monthscomparedwithcontrols,exceptforserotype19F.HighersalivaryIgAlevelsremainedpresent for most serotypes in the 2+1-dose group until 24 months, but not in the 2-dose group. Salivary IgA more than IgG, increasedafterdocumentedcarriageofserotypes6B,19Fand23FIncontrasttoIgG,salivaryIgA-levelswerecomparable withserum,suggestinglocalIgA-production. Conclusions:PCV7vaccinationresultsinsignificantincreasesinsalivaryIgGandIgA-levels,whicha
