Abstract Background:Robusthardwareandsoftwaretoolshavebeendevelopedindigitalmicroscopyduringthepast yearsforpathologists.Reportshavebeenadvocatedthereliabilityofdigitalslidesinroutinediagnostics.Wehave designedaretrospective,comparativestudytoevaluatethescanningpropertiesanddigitalslidebaseddiagnostic accuracy. Methods:8pathologistsreevaluated306randomlyselectedcasesfromourarchives.Theslideswerescannedwith a20×Plan-Apochromatobjective,usinga3-chipHitachicamera,resulting0.465μm/pixelresolution.Slide managementwassupportedwithdedicatedDataBaseandViewersoftwaretools.Pathologistsusedtheiroffice PCsforevaluationandreachedthedigitalslidesviaintranetconnection.Thediagnosticcoherencyanduncertainty relatedtodigitalslidesandscanningqualitywereanalyzed. Results:Goodtoexcellentimagequalityofslideswasrecordedin96%.Inhalfofthecritical61digitalslides,poor imagequalitywasrelatedtosectionfoldsorfloatings.In88.2%ofthestudiedcasesthedigitaldiagnoseswerein fullagreementwiththeconsensus.Outoftheoverall36incoherentcases,7(2.3%)weregradedrelevantwithout anyrecordeduncertaintybythepathologist.Excludingthenon-fieldspecificcasesfromeachpathologist’ srecord thisratiowas1.76%ofallcases. Conclusions:Ourresultsrevealedthat:1)digitalslidebasedhistopathologicaldiagnosescanbehighlycoherent withthoseusingopticalmicroscopy;2)thecompetencyofpathologistsisafactormoreimportantthanthequality ofdigitalslide;3)poordigitalslidequalitydonotendangerpatientsafetyastheseerrorsarerecognizablebythe pathologistandfurtheractionsforcorrectioncouldbetaken.
