我院2004年抗生素不合理处方分析 ［摘要］ 目的 了解我院门、急诊抗生素不合理使用情况。 方法 随机抽取2004年度门、急诊处方，根据临床药 理学 知识及 文献 资料，对不合理的抗生素用药处方进行 分析 ，并加以分类统计。结果 共审核处方21336张，其中抗生素处方7468张，存在不合理用药897张，分别在溶媒使用、给药方案、药理作用、合并用药、重复用药、剂量不足以及使用不当方面存在 问题 。结论 我院抗生素使用基本符合抗菌原则但仍需进一步提高；处方审核将有助于促进抗生素合理使用。 ［关键词］ 门、急诊处方；抗生素；合理用药；不合理处方分析 Analysis of antibiotic unreasonable prescription for outpatient and emergency in our hospital in 200 ［Abstract］ Objective To learn the unreasonable antibiotic prescription.Methods To select prescription of Outpatient and Emergency at random in 2004.To analyze the unreasonable antibiotic prescription and performs to classify the statistics according to clinical Pharmacology knowledge and literature material.Results Altogether verifies prescription 21，336，Antibiotic prescription was 7，468，nonreasonable usage was 897，Separately in solvent use，for medicine plan，pharmacological action，combination，repetition，dosage insufficiency as well as use not when the aspect has the problem.Conclusion The usage of the antibacterial agents basically conforms to the antibacterial principle，but it still has further to enhance；The prescription verification will be helpful to the promotion the usage of the antibacterials. ［Key words］ the prescription of outpatient and emergency；antibacterial agents；reasonable usage；analysis of antibiotic unreasonable prescription 随着医疗水平的提高及新药种类的增多，临床上多药合用屡见不鲜，尤以抗生素为甚。抗菌药物可以防病治病，但在正常剂量下，也可引起毒性反应和二重感染，使用不当，还会使体内某些致病菌产生耐药性，造成感染难以控制的后果。我院作为一所二甲滨海地区中心 医院 ，对抗生素的合理使用非常重视，为促进抗生素的合理使用，我院临床药师定期对门、急诊处方随机进行抽查，统计分析抗生素的不合理 应用 ，并将结果在全院宣传，以期达到合理用药的目的。 1 资料与方法 随机抽取2004年度门、急诊病人处方，共21336张。根据药品说明书、公开发表的国内外文献及公开出版的书籍，对抗生素处方的用法、用量、配伍、合并用药等情况进行系统分类和统计分析。 2 结果 21336张处方中，抗生素处方共有7468张，占查方总数的35%，某些不合理处方中存在多项不合理之处，经统计，共有897处不合理使用。具体情况见表1。表1 抗生素使用概况（略） 3 讨论 临床上抗生素使用不合理，不但 影响 治疗 的效果，而且会引起严重的不良反应，并导致细菌产生耐药性。抗生素的不合理应用在美国占24%~66%，在我国为25%~56%［1］。本年度审核处方，不合理的抗生素处方占抗生素处方的12%（897/7468），低于平均水平，由表1可看出，我院抗生素使用大多单药使用（82%），二药联用为13.99%，符合抗菌治疗原则。不合理之处主要在给药方案、重复用药、溶媒不当、药理拮抗、毒性相加、剂量不足及使用不当等方面，见表2。表2 不合理用药问题一览表（略）
