耳背静脉放血治疗无先兆偏头痛发作期的即刻镇痛效应和其机制研究.pdfVIP

提 要 目的：观察耳背静脉放血对无先兆偏头痛发作期患者的即刻镇痛效应，并通过实 验研究以初步探讨其作用机制。 方法：临床试验将 40 例符合纳入标准的无先兆偏头痛发作期患者，按随机数字 表法随机分为治疗组和对照组各 20 例，治疗组采用耳背静脉放血，对照组口服麦角 胺咖啡因，通过观察治疗前后患者 McGill 量表积分的变化以评价即刻镇痛效应。动 物实验将 30 只新西兰家兔按随机数字表法随机分为正常组、模型组及放血组，每组 10 只。放血组造模 30min 后家兔耳缘静脉放血，即刻观察家兔的痛阈值，并检测家 兔血浆、脑干的SP、CGRP含量。 结果：①耳背静脉放血对无先兆偏头痛发作期患者有非常明显的即刻镇痛疗效（P ＜0.01），与对照组比较无显著性差异（P＞0.05）；②耳缘静脉放血对偏头痛家兔模 型有非常明显的即刻镇痛作用（P＜0.01），明显降低了血浆及脑干中 SP、CGRP 的含 量（P＜0.05）。 结论：耳背静脉放血对无先兆偏头痛发作期患者有较好的即刻镇痛疗效，其镇痛 机制可能与降低血浆及脑干的SP、CGRP含量有关。 关键词 无先兆偏头痛；耳背静脉放血；偏头痛家兔模型；效应机制 Clinical observation of the instant analgesic effect of the migraine without aura patients in stage of attack by ear vein’s bloodletting and research its action mechanism Specialty: Acupuncture and moxibustion Author: Liu Yingying Tutor:Yang Dianhui Abstract Objective:To observe and evaluate the clinical instant analgesic effect of the migrainewithout aura patients in stage of attack by bloodletting via ear vein ，and through the animal experiment provisionally discusses its action mechanism． Method: By the way of random and control trial，40 migraine without aura patients in stage of intervals were randomly assigned into treatment group and control group，the post aur venous blooding group with 20 patients and control group in which 20 patients took orally Ergotamine caffeine．Observe the change of McGill gauge integral before and after the patients’treatment．By the same way of random and control trial，30 New Zealand rabbits were randomly averaged into three groups，common group、model group and blooding group．Bled the two ears of rabbits in blooding group，observed the change of pain threshold，and tested the content of SP、 CGRP in rabbits’ blood plasma and brainstem． Result:① The post aur venous blooding have the patent clinical analgesia effect of
