3250例消化性溃疡电子胃镜检查研究【摘要】 目的 了解新疆地区消化性溃疡(PU)的发病情况及内镜下特点。方法 分析新疆医科大学第一附属医院2000年12月至2005年2月间胃镜检查消化性溃疡的临床资料。结果 在18560例胃镜检查中，检出3250例PU，检出率为17．5%，其中，胃溃疡(GU) 2150例，十二指肠溃疡(DU)884例，复合性溃疡(CU)216例。结论 PU的患者男性多于女性；DU多于GU；DU好发的部位是十二指肠球前壁；GU的好发部位是胃角和胃窦；PU合并疾病多；四季的检出率相差不大。? 【关键词】 消化性溃疡；胃镜；十二指肠溃疡；胃溃疡；复合性溃疡?? Results The detection rate of PU was 17.5%,the duodenal ulcer(DU)account for 66．2% ,gastric ulcer(GU)27．2%, compound ulcer (CU) 6．6%.Conclusion The PU patient males were more than the females; DU rating were more than the GU; The uker incidence of stomach and sinus were more than other station, and the merge disease of PU were much and the detection rate of four seasons detection was not significant difference.? 【Key words】Peptic ulcer; Gastroscopic; Duodenal ulcer; Gastric ulcer; Compound ulcer 消化性溃疡（peptic ulcer，PU）是指出现消化道症状并经胃镜及病理检查排除恶性病变者[1]。消化性溃疡病呈世界性分布，约有5%～10%的人在其一生中某一时期罹患此病，其发病率近年来有明显增高趋势[2]。为了解新疆地区溃疡病的某些流行病学特点，本文对新疆医科大学第一附属医院2000年12月至2005年2月间，经胃镜检查诊断为消化性溃疡(PU)的3250例病例进行研究分析，分析其性别、年龄及职业等分布和好发季节、溃疡分布部位、溃疡大小及检出数目等特点，现报告如下。? 1 临床资料? 1．1 一般资料 2000年12月至2005年2月，新疆医科大学第一附属医院内镜室对有上消化道临床症状的18560例患者进行内镜检查，共检出消化性溃疡3250例，其中男2150例，女1100例，年龄12～79岁，平均46．6岁。对其中十二指肠溃疡(duodenal ulcer，DU)、胃溃疡(gastric ulcer，GU)及复合性溃疡(complex ulcer，CU)的检出率、性别、年龄、发病部位及与季节的关系进行综合分析。? 1．2 检查方法 采用日本奥林巴斯V70型电子胃镜，所有资料均由电脑胃镜工作站储存。多发性溃疡以最大病灶所在部位进行统计，幽门前及幽门管溃疡归于胃溃疡中。吻合口溃疡和溃疡型胃癌不在统计之列。? 1．3 统计学方法 应用SPSS 10．0统计软件处理有关数据，采用χ2检验，P＜0．05认为有统计学差异。? 2 结果? 2．1 PU检出率 18560例患者共检出PU 3250例，检出率317．5%，其中男2150例，女1100例；十二指肠溃疡(DU)2150例，占66．2%，胃溃疡(GU)884例，占27．2%，复合性溃疡(CU)216例，占6．6%。
