评价多个肿瘤标志物在非小细胞肺癌临床中应用价值【摘要】目的：评价血清血管内皮生长因子（VEGF），细胞角蛋白片段（CYFRA21-1），血清癌胚抗原（CEA）及骨桥蛋白（OPN）在非小细胞肺癌临床中的应用价值。 方法：采用酶联免疫吸附法ELISA对45例各期非小细胞肺癌及30例肺良性病变患者进行CEA，VEGF，CYFRA21-1，OPN检测，并测定单个或四种肿瘤标志物联合检测对非小细胞肺癌诊断的敏感性及特异性。结果：在非小细胞肺癌患者中上述各肿瘤标志物的血清水平均高于肺部良性病变者，差异有统计学意义（P0.05）。CYFRA21-1对鳞癌敏感性及特异性最高，分别为（69.4%和86.5%）。CEA腺癌敏感性及特异性为最高，分别为（70.3%和91.6%）。联合检测的敏感性及特异性分别为（84.0%和85.9%），较单项肺癌标志物敏感性明显提高（P0.05），特异性下降则不明显。结论：血清CEA，CYFRA21-1，OPN及VEGF水平有助于非小细胞肺癌的诊断及肺部良恶性疾病的鉴别。CYFRA21-1对于肺鳞癌的敏感性和特异性最高，CEA对于肺腺癌诊断的敏感性及特异性最高，对于非小细胞肺癌的病理类型的判定有重要作用。四种标志物联合检测较单项肺癌标志物敏感性明显提高（P0.05），对于临床非小细胞肺癌的诊断有一定的应用价值。 【关键词】非小细胞肺癌；肿瘤标志物；血管内皮生长因子；骨桥蛋白；细胞角蛋白片断 文章编号：1009-5519（2007）23-3484-03 中图分类号：R73 文献标识码：A Evaluation of multiple tumor markers in non-small cell lung cancer HU Chun-xia,ZHANG Jin (Department of Oncology,The Second People’s Hospital of Liangungang，Jiangsu 222006，China） 【Abstract】Objective：To evaluate the serum tumor markers of CEA,CYFRA21-1,OPN andVEGF in the non-small cell lung cancer.Methods：The levels of CEA CYFRA21-1, OPN andVEGF in 45 patients with non-small cell lung cancer in different TNM stages and 30 patients with benign lung disease were measured with ELISA（Enzyme-linked immunosorbent Assay）method respecitively and were worked out their sensitivity and specificity were respectively.Results：The serum concentrations of the four tumor markers in patients with non-small cell lung cancer were higher than those of benign lung disease.The sensitivity and specificity of CYFRA21 were the highest for squamous carcinoma (69.4% and 86.5% respectively) and those of CEA were the highest for adenocarcinoma(70.3%and 91.6% respectively).The sensitivity and specificityofthe four combined tumor markers in non-small cell lung cancer were (84.0% and 85.9% respectively),compared with the single serum tumor marker,the sensitivity increased obviously (P0.05)，but the specificity decreased slightly.
