识别和管理腹膜透析患者的营养不良 宁雅娴 王俭勤((( 兰州大学第二医院肾病科，甘肃兰州，730030 摘要：蛋白质-能量营养不良（PEM）在腹膜透析（PD）患者中是非常普遍存在的，并且是一个强有力的死亡率预测因子。各种因素可以导致PD患者PEM和相关的能量消耗（PEM/ W），其中持续的炎症状态和糖尿病已被确定为导致PEM/ W的两个主要原因。已有研究报告说，部分方法可用于早期的PEM/ W检测，并提供适当的干预措施。虽然PD患者营养状况的评估方法已大幅度得到改善，但没有一种单一的方法来评估营养状况。PD患者的PEM/ W是较为复杂的，可以通过多个不同的的营养指标来进行评估。因此，传统方式如营养支持等与非传统的策略，如食欲的兴奋剂，抗炎症反应药物制剂等相结合，从而提供一些更好的改善PEM/ W的方法。本次简要讨论了PEM/ W的原因，并提供用于识别和管理不同原因导致PEM/ W的方法. 关键字： 蛋白质-能量营养不良（PEM）、腹膜透析（PD）、炎症反应、糖尿病 Abstract: Protein - energy malnutrition (PEM) is very prevalent in peritoneal dialysis (PD) patients, and is a strong predictor of mortality. The various factors can lead to the PD patients PEM and related energy consumption (PEM / W), wherein the sustained inflammatory state and diabetes has been identified as leading to two of the main reasons of the PEM / W. It has been reported that part of the method can be used for early detection of PEM / W, and provide appropriate interventions. Although the method of assessment of the nutritional status of PD patients has been greatly improved, but no single method to assess nutritional status. PD patients with PEM / W is more complex, and can be evaluated by a number of different indicators of nutrition. Therefore, the traditional methods such as nutritional support and non-traditional strategies, such as appetite stimulants, anti-inflammatory drugs preparations combination to provide a better method to improve the PEM / W.
