E-钙粘素在结直肠癌中表达及其及淋巴结微转移关系 　 作者:张翼飞 姜立新 郑海涛 　　【摘要】 目的 检测结直肠癌组织中E-钙粘素的表达及其淋巴结微转移,探讨两者间的关系。方法 以CK20 mRNA为标记物,应用RT-PCR对60例结直肠癌根治性手术切除的淋巴结标本共782例进行检测;免疫组化法(SP法)检测结直肠癌组织、癌旁组织、淋巴结微转移及正常对照组中E-钙粘素的表达。通过二者的综合分析,试图发现抑癌基因E-钙粘素在结直肠癌淋巴结微转移的生物学行为中的作用。结果 CK20法检出45例(75.0%)患者有淋巴结转移,阳性淋巴结总数为436个(55.8%);常规病理学检出34例(56.7%)患者有淋巴结转移,检出阳性淋巴结数为356个(45.5%);两组比较,差异具有统计学意义(χ2=16.72,Plt;0.05)。11例检出淋巴结微转移患者癌组织中6例(54.5%)E-Cadherin表达阴性,而15例未检出淋巴结微转移癌组织中仅有1例(6.67%)表达阴性,两者比较差异有统计学意义(χ2=4.160,P=0.041)。结论 E-Cadherin的表达减弱或消失参与了结直肠癌淋巴结微转移的发生。 　　【关键词】结直肠癌;E-钙粘素;淋巴结转移;逆转录聚合酶链反应 　　【Abstract】 Objective Purpose To detect the expression of E-cadherin in colorectal carcinoma and lymph node micrometastases,and investigate their relationship.Methods 82 cases of lymph node simple was dectected with RT-PCR method using CK20 mRNA acridinium ester.in screening colorectal carcinoma lymph node micrometastases. E-Cadherin expression analysed byimmuno-histochemical staining (the SP method) in colorectal cancer tissue, tissue nearby cancer, the normal control tissue. Results In 60 cases of colorectal carcinoma,the expression of E-Cadherin was 71.67%.Number of positive nodes was 55.8%, the expression of E-cadherin was 56.7%(34 cases) and number of positive nodes was 45.5%(356 cases) through routine pathological examination. The low level expression of E-Cadherin was positively correlated with the Dukes staging, serosa infiltration, lymph node and liver metastasis of colorectal carcinoma(χ2=16.72,Plt;0.05).The expression of E-Cadherin was negative in 6 of the 11 patients with lymph node micrometastases,and in 6.67%1 of the 15 patients without lymph node micrometastases(χ2=4.160,P=0.041).Conclusion The negative expression of E-Cadherin may contribute to lymph node micrometastases in colorectal carcinoma. 　　【Key words】 　　colorectal carcinoma; E-Cadherin; Lymphatic metastasis; Rt-pcr 　　 　　结直肠癌是我国最常见的恶性肿瘤之一。每年有10%的恶性肿瘤患者死于结直肠癌[1,2]。术后复发和转移是影响结直肠癌预后的最主要因素之一。在实施了根治性手术的结直肠癌患者中,仍有20%~50%的患者死于术后肿瘤的局部复发和转移[3,4],有研究认为
