全髋置换术后早期并发深静脉血栓形成多因素分析 　　　　　　　　　作者：冯永洪，臧学慧，张利，黄中强，莫景生，吴旭辉【摘要】 目的：探讨多个因素对人工全髋关节置换术早期并发深静脉血栓形成(DVT)的影响及相应的预防和处理对策。方法：分析1 208例首次全髋关节置换术病例，分析年龄、性别、体重、病种、合并症、假体固定方式、手术时间、输血、麻醉方式、患肢被动训练、术后防血栓药物等多因素对DVT的影响进行统计学分析。结果：1 208例中发生早期并发DVT共93例，其中男39例，女54例，发生率为7.69%；性别、体重、病种、手术时间对DVT的发生无明显影响（Pgt;0.05）；年龄、合并症、假体固定方式、输血、麻醉方式、患肢被动训练、术后防血栓药物对DVT的发生有明显影响（Plt;0.05）。结论：年龄、合并症、假体固定方式、输血、麻醉方式、患肢被动训练、术后防血栓药物对DVT的发生有明显影响，我们应该认真对待积极预防。 【关键词】 全髋关节置换；早期；深静脉血栓形成；多因素分析 　　Multifactors Analysis for Early Deep Venous Thrombosis After Total Hip Replacement 　　Abstract：Objective To explore the effects of multifactors for the early deep venous thrombosis (DVT) of total hip replacement and summarise some measures to prevent and treat the early DVT.Methods 1 208 cases of primary total hip replacement operation were analyzed.The statistical indexes included age,sex,body weight,disease category,other system diseases,prosthetic fixation,operative time,blood transfusion,anesthesia,postoperative paasive functional exercise,antithrombotics.Multifactors Analysis was made for those factors and summarization were made for the effects of each factor for the early DVT.Results 93 cases had DVT of hip joint included 39 male cases and 54 female cases.The occurrat of DVT was 7.69%.There had no statistical signification of the analysis for factors of sex,body weight,disease category,operative time（Pgt;0.05）.There had statistical signification of the analysis for factors of age,other system diseases,prosthetic fixation,blood transfusion,anesthesia,postoperative paasive functional exercise,antithrombotics（Plt;0.05）.Conclusion The factors of age,other system diseases,prosthetic fixation,blood transfusion,anesthesia,postoperative paasive functional exercise and antithrombotics are the effective factors for the early deep venous thrombosis (DVT) of total hip replacement.We must do something to prevent and treat the early DVT seriously. 　　 　　Key words：Total hip replacement;Early;Deep venous thrombosis;Multifactors analys
