髌骨骨折分型及治疗策略改进的疗效分析 作者：刘宝平，王涛，张鹤令，李克文，杨杰山 作者单位：青海大学附属医院骨科，青海 西宁 810000 【摘要】 目的 探讨髌骨骨折分型及治疗的临床效果，寻找治疗髌骨骨折的规范化术式。方法 在对1998～2000年(A组)48 例髌骨骨折疗效欠佳病例分析的基础上，对2000～2006年(B组)109 例髌骨骨折的分型及治疗策略改进，做回顾性总结分析。将髌骨骨折分为七种类型，对109 例髌骨骨折进行分型，对应选择合理的治疗方法并观察其疗效。结果 术后AB两组均随访12～36个月，平均26个月，按陆裕朴等的疗效评定标准进行评定，A组：优良率89.6％，优比率75％。B组：优良率96.3％，优比率86.2％。结论 在传统髌骨骨折分型的基础上重新对其归类、补充以及改进治疗方法，在保证原有髌骨骨折治疗优良率的同时，可明显提高优良率及优比率。 【关键词】 髌骨骨折；分型；策略；改进；内固定 Efficacy Analysis of Improvement on Classification and of Treatment Strategies for Patellar Fracture LIU Baoping，WANG Tao，ZHANG Heling，et al (Department of Orthopaedics，the Affiliated Hospital of Qinghai College，Xi′ning 810000，China) Abstract：Objective To explore a classification method and its clinical efficacy of treatment for patellar fractures.Methods Based on the 48 cases(A group)with patellar fracture between 1998 and 2000 whose therapeutic efficiencies are not very good，the 108 cases(B group)with patellar fracture between 2000 and 2006 were improved on the classification and therapeutic method.According to respective analysis，the patellar fracture was divided into ⅤⅢ types，select reasonable therapeutic methods and observe the therapeutic effect.Results All patients were followed up from 12 months to 36 months with 26 months on the average.According to Lu yupu′s evaluation criteria of therapeutic efficiency，A group：the excellent and good rate was 91.67％，the excellent rate was 77.08％，and the poor rate was 6.2％.B group：the excellent and good rate was 97.20％，the excellent rate was 86.1％，and the poor rate was 0％.Conclusion To recapitulate and supply the type based on the traditional type of patellar fracture and to improve its therapeutic methods，not only ensure original rate of excellent and good，but also significantly increase the rate and the respective rate of excellent and good，and reduce the rate of fair and poor.
