904 堂塞壁 1o年 月第25卷第8期 RadiolPractice，Aug2010，Vol25，No．8 腹 部影像 学 口 多层螺旋 CT尿路成像在尿漏中的诊断作用 陈亮，王秋霞，胡道予，郑楠楠，李华玲 【摘要】 目的：探讨 多层螺旋CT尿路成像 (MSCTU)在 肾盂 、输尿管 、膀胱尿漏中的诊 断价值 。方法 ：对 15例经临床 证实发生尿漏的患者进行三期 MSCTU检查，采用多种CT图像后处理技术(MPR、MIP、VR)综合分析其MSCTU表现特 点。结果：15例尿漏患者中，肾盂尿漏 5例 ，输尿管漏 8例 ，膀胱漏2例 。尿漏患者 MsCTu 的排泄期表现为对比剂外溢 ， MIP、VR并结合 MPR可以清晰显示漏 口的部位及范围，并可显示伴发的肾实质破裂，肾包膜下血肿 、肾周间隙积液、原发 肿瘤(宫颈癌转移瘤侵及右输尿管)及泌尿系先天畸形等。结论 ：多层螺旋 CT尿路成像 (MSCTU)能清晰显示尿漏的部 位 、漏 口并初步判 断尿漏发生的原 因，可以作为诊断尿漏的首选方法。 【关键词】 cT尿路造影 ；尿漏；体层摄影术 ，X线计算机 【中图分类号】R814．42；R695 【文献标识码】A 【文章编号】1000—0313(20l0)08—0904—04 Clinicalapplicationofmulti—slicespiralCTurographyinurineleakage CHENLiang，WANGQiu—xia，HU I)ao—yu，eta1．I)e— partmentofRadiology，TongjiHospital，TongJiMedicalCollege，HuazhongUniversityofScienceandTeehnology，Wuhan 430030，P．R．China [Abstract] Objective：Toexplorethediagnosticvalueofmulti—sliceCT urography (MSC1、U)intheurineleakage． Methods：15casesofurineleaksconfirmedbyclinicaldiagnosisunderwentspiralCT plainscanningandtriphasicdynam ic enhancementscanning．TheCTfeaturesonaxialimagesandmultiplanarreformation(MPR)，maximum intensityprojection (M IP)andvolumerendering (VR)werereviewed．Results：Thebreakageinrenalparenchyma，subcapsularhematomaof kidney，prerenalfasciathickeningappearedin5caseswithpelvisleakaffectedbytrauma．Theexcretoryphasecoulddetect theleakageclearly，M IP andVR couldshow thelakeofcontrastagent．Of8caseswith ureteralleakage，1casebytrauma damagingtherightureter，1casebyEW SI damaging therightureter，and 1caseinvadedbycervicalcancermetastasis，a largenumberofretrorenalspacefluidaccumulation，thespillsofcontrastagentinexcretoryphase，andthecontrastagent depositioninhydropscouldbeseen，sagittalMPR andVR clearlyindicatedtheleakage，1casewiththeleftpelvicectopic kidneyandeetopicureteropeninginthevag
