医学临床研究 2o1o年 8月 第 27卷 第 8期 JClinRes，Aug．2010，y 7，No8 · 论著 · 腹腔镜手术联合药物治疗子宫内膜异位症 117例临床分析 廖金兰 陆桂玲 (南方 医科大学附属广东省江 门市新会人民医院妇产科，广东 江 门 529100) [摘要]【目的】探讨腹腔镜手术联合孕三烯酮或促性腺激素激动剂 (GnRH—a)治疗子宫内膜异位(内异 症)的临床疗效。 【方法】对 117例 内异症患者进行腹腔镜手术治疗，术后随机分为孕三烯酮组、GnRH～a组和 对照组，比较三组的症状缓解率、妊娠率、术后复发率及药物副反应等情况。 【结果】孕三烯酮组和 GnRH—a 组的症状缓解率、妊娠率均较对照组 明显提高，复发率明显降低 ；孕三烯酮组常见的副反应有肝功能损害、阴 道 出血 、痤疮 、体 重增加等 ，而 GnRH—a组 常见 副反应 主要 为 绝经期症状。 【结论】腹腔镜 手 术联合 药物 孕三 烯酮或 GnRH—a治疗 内异症对提高症状缓解率、妊娠率及降低复发率有重要 的作用，但两种药物均有一定的 副作用 ，常见 的有肝功能损 害、阴道 出血及绝经期症状等。 [关键词] 子宫内膜畀位症 ／外科学； 子宫内膜异位症 ／药物疗法； 外科手术 ，腹腔镜 ClinicalAnalysison LaparoscopicOperationCombined with M edication fortheTreatmentof 117CasesofEndometriosis LIAO Jin—lan， LU Guiling (Departmentof Obstetricsand Gynecology，XinhuiRenminHospitalofJiangmenCity，SouthernMedicalUniversity，Cruangdong529100， Ckina ) [Abstraet][Objective]Tostudytheclinicalefficacyoflaparoscopicoperationcombinedwithgestrinoneor GnRH—aforthetreatmentofendometriosis．[Methods]A totalof117patientswithendometriosisweretrea— tedbylaparoscopy．Aftertreatment，they weredividedintogestrinonegroup，GnRH—a group and control group．Remissionrate，pregnancyoutcome，relapserateand drug sideeffectamongthreegroupswerecom— pared．[Results]Comparedwiththecontrolgroup，theremissionrateandpregnancyoutcomeincreasedand therelapse ratedecreased obviously in gestrinonegroup and GnRH—agroup． The common sideeffectsin gestrinonewereliverdamage，vaginalbleeding，acne，andweightgain，etc．Andthecommon sideeffectsin GnRH—agroupmainlyweremenopausalsymptoms．【Conc1usion】Laparoscopicoperationcombinedwithgestri— noneorGnRH一3forthetreatmentofendometriosishasan importantrolein increasingtheremissionrateand pregnancyrateanddecreasingtherelapserate．Butthetwodrugshavecommonsideeffectssuchasliverdam— age，vaginalbleedingandmenopausalsymptoms，etc． [Keywords] endometriosis／SU； endometriosis／DT； surgicalprocudures，laparoscopic [中图分类
