Methods：A sum of 62 patients with MVR performed due to chronic rheumatic valvular disease were taken and divided into two groups according to the form of operations: the MVRP group of 33 cases with the subvalvular preservation and the MVRNP group of 29 cases with the subvalvular structures revoved. The BNP level of blood plasma before, 7 days and 6 months after the operation, were measured respectively. Results：Significant shrinkage of BNP was found between MVRP groups and MVRNP groups.Conclusions：Compared with the conventional MVRNP, operations with the posterior mitral preserved are more favourable in improving the configuration and function of left atria. 【Key words】 mitral valve replacement subvalvular structure BNP 二尖瓣置换术（mitrial vavle replacement，MVR 首次出现的英文缩写补充中英文全称，下同）为外科治疗风湿性心瓣膜病二尖瓣病变的主要方法，然而在手术时是否保留瓣下结构目前尚存有争议。本研究通过测定患者手术前、后血清脑钠素（Levels of serum brain natriuretic peptide，BNP）的浓度，评价保留二尖瓣后瓣下结构的二尖瓣置换术是否有利于改善左室功能。 1 资料与方法 1.1 一般资料 选取2008年7月～2010年5月间因慢性风湿性心瓣膜病在我院施行二尖瓣置换术MVR或二尖瓣、主动脉瓣联合置换术（double valve replacement，DVR）术的连续病例62例，男性21例，女性41例，年龄（42.5plusmn;8.6）岁（21～69岁），其中保留二尖瓣后瓣下结构二尖瓣置换术（mitral valve replacement with the sub
